Aging and Disability Resource Center Information and Assistance Follow-Up Policy – Wisconsin

This document contains best practice examples and ideas for follow-up policies. Aging and disability resource centers (ADRCs) are required to have a written policy describing when and how they will follow-up with people that contact them for information and assistance. The policy helps determine outcomes and provides additional assistance when needed.

Gateway Services Consumer Satisfaction Survey & Report - Georgia

This report and sample survey were used to evaluate information and assistance services provided by GA Area Agencies on Aging (AAA). The report summarizes the responses and lists recommendations for measuring and improving consumer satisfaction with this aspect of AAA services. The Quality Assurance Team of the Division of Aging Services mailed the survey to users such as consumers and their caregivers, case managers, friends and family members. They received 840 responses.

MinnesotaHelp.info Data Management Services: RFP

The MN Board on Aging, the agency with primary responsibility for MinnesotaHelp.info, issued this RFP with the goal of contracting an organization to maintain the database underpinning the website. The site is an online directory of services helping people in Minnesota find human services, information and referral, financial assistance, and other forms of help. Proposals must be submitted by Friday, May 30, 2008.

Retooling for an Aging America: Building the Health Care Workforce

This book and corresponding materials present a strategy for improving the health care workforce serving older adults via expanded education and training, models of care, and public/private programs. The committee proposes a three-prong approach: 1)Enhance the geriatric competence of the entire workforce; 2)Increase the recruitment and retention of geriatric specialists and caregivers; 3)Improve the way care is delivered. Real Choice Systems Change and Aging and Disability Resource Center/Area Aging Grant Solicitation FY 2008

CMS has available approximately $8 million in funding to continue to support States' efforts to address complex issues in long-term care reform. In addition, CMS was also awarded $5 million for Aging and Disability Resource Center /Area Agencies on Aging grants. Applications are due no later than July 17, 2008. Aging and Disability Resource Center Pilot Project Request for Proposals - Iowa

ADRC Memorandum of Understanding with Partners - Washington

The Pierce County ADRC in Washington State uses this Memorandum of Understanding (MOU) with its partners to establish collaborative, mutual agreements. Included in the MOU are project descriptions, goals and services explanations, mutual agreement terms, including a requirement for cross-training, and confidentiality agreements.
