The 2008 national Aging and Disability Resource Center meeting took place in October in Boston. Find here the presentations and handouts from the workshops and plenary sessions. Topics ranged from nursing home diversion to quality to information flow and technology.

This poster and brochure tell older adults, caregivers and individuals with disabilities living about information and referral services available at the Arkansas aging and disabilities resource center (ADRC). Use these as examples of how other ADRCs might consider spreading the word about their programs and services.

This document includes the “Arizona Links” common intake form as well as the instructions for completing the form. This common intake is intended be filed on behalf of a consumer, who is a resident or intends to become a resident of Arizona, and who is seeking or requiring direct services in the state.

The MD Department of Aging envisions the Maryland Access Point (MAP) website as a place where older adults and people with disabilities can find information about services and resources in the state. MAP supports the work of local Aging and Disability Resource Centers (ADRCs). Use this closed RFP as an example if your program is searching for assistance in website development and maintenance.

Seven states received a share of over $8 million in federal grants to increase awareness of home-and community-based long-term care options for people leaving hospitals who otherwise may enter a nursing home. Over $7 million of the awards will be used to develop person-centered hospital discharge planning models. An additional $574,613 will supplement grants from FY07 to Wisconsin and Alaska so they can continue their Person-Centered Planning Implementation programs for another two years.

SCAccess provides this one-stop website for finding aging and disability resources, applying for benefits, and learning about rights. One section contains a directory for finding and tips for hiring the right personal care worker. Another helps individuals connect with information specialists at each Area Agency on Aging. Consumers can also download commonly used documents and applications. Finally, the site is integrated with agency case management tools.

Staff at the Hawaii ADRC were oriented to the ADRC model and trained in vital customer service and communications skills using these materials. The outline lists the objectives and time needed for training exercises. Two curriculum grids, one for ADRC orientation and one for customer service skills, list training areas, outcomes, and methods. Each grid contains materials for both paid staff and volunteers.

This graphic shows how information requests from or about people with long-term care needs are handled in Ohio. The illustrated processes include the information request, intake and referral, assessment, and follow-up and evaluation.

This booklet, through Arkansas’ “Choices in Living” aging and disabilities resource center, profiles the home and community-based long-term care (LTC) options available in the state. Older adults, people with disabilities, families and support workers can determine which of the five profiled programs are best for them based on eligibility criteria and scope of coverage.

This report presents findings from Oct ‘05 – Oct ‘07, which includes the first nine months of strategic planning for the FY05 and FY06 Systems Transformation (ST) Grants and the subsequent initial 15-months of implementation (through Oct ‘07) for the FY05 ST Grants. The report focuses primarily on documenting grantees' experiences during the start-up period and grantees' progress during early implementation and suggests early trends and findings.
