Complex Care

Through support from the Patient-Centered Outcomes Research Institute, CHCS has published a Patient-Centered Complex Care Research Agenda to help involve people with lived experiences with complex care in the research process. The Better Care Playbook spoke to two individuals who helped develop the Research Agenda to learn how involving people with lived experience in research can help strengthen the complex care evidence base.

Through the Better Care Playbook, CHCS has organized a virtual panel of innovators to discuss challenges in applying evidence when designing and refining complex care program. This blog post shares key themes from the panel's discussion at the 2021 “Putting Care at the Center”conference, hosted by the Camden Coalition’s National Center for Complex Health and Social Needs. During this discussion, panelists shared their experience translating research findings into decision making for complex care program design and improvements.

Health care stakeholders can use this Playbook Collection to understand recent evidence behind telehealth-delivered interventions and learn about practical strategies to support effective implementation of these programs for people with complex needs.

Building Complex Care Programs: A Road Map for States is a guide for state leaders in establishing and advancing complex care programs. This guide from the National Governors Association Center for Best Practices (NGA Center) Health Division includes lessons learned from their work with states and effective practices gleaned from multiple pioneering state and local complex care initiatives.
