Personal Profiles: Older Adults Who Have Transitioned from Private Automobile to Public Transit Use

NCST developed this resource to provide background information on mobility for older adults with disabilities and profiles five individuals who have successfully made the transition from personal vehicle to public transportation. The packet includes a profile template and a model release form that service providers can use to create personal profiles of senior transit users in their own communities.

IRIS: Wisconsin’s Self-Directed Supports Program - Website and Outreach Material

IRIS is a self-directed, long-term care option for elderly people and adults with disabilities. This Medicaid waiver program sponsored by the Wisconsin Department of Health Services is available in the 47 counties where Family Care operates. Their website provides information about the program, forms, FAQs, enrollment information and FSA and Participant Handbook.

Maryland Bridge Subsidy Demonstration Program

The Demonstration aims to provide 75 to 100 low-income residents with rental assistance for up to 3 years as they wait for permanent housing assistance. A process map showing how a subsidy is provided, marketing materials, sample contracts, and several worksheets for gathering unit and participant information and determining assistance eligibility and amount are given here.

Informational Materials for the MBIWD Project - Ohio

In this collection of informational materials, Ohio outlines what its Medicaid Buy-In for Workers with Disabilities (MBIWD) program is. Documents include an explanatory booklet, flyer and fact sheets. A website link with the government site that houses these documents, application forms and additional information is also included. While this material was intended for consumer use, it could serve as a guide to other states contemplating or running a similar MIG program.

The Arc of Spokane Advocacy & Family Support - Website & Newsletters

Through advocacy and support services, the Arc of Spokane promotes independence, choice, and improved quality of life for people with developmental disabilities and their families in Eastern Washington. This edition of their newsletter discusses state and county advocacy and politics, words from staff, tax advice, upcoming events, and program information.
