Should a state use the cost reimbursement or milestone-outcome payment options in their Ticket to Work (TTW) programs? Vermont commissioned this study to answer this question and to predict future TTW income for the state and for employment network partners.

Support workers use this tool as a preliminary eligibility test for clients that wish to apply for Medical Assistance for Workers with Disabilities (MAWD). Once initial screening is passed, clients receive an application and the questionnaire is sent for eligibility verification.

This report presents the results of a survey of state developmental disabilities agency officials regarding the determination of initial eligibility/level of care and the “annual level of care” for services furnished under Medicaid waiver and ICF/MR programs. Forty-seven (92%) states provided information on state waiver eligibility criteria and practices. The report describes state level of care assessment approaches and provides legislative cites and contact information for state staff.

Agencies or programs can use this worksheet to generate and understand employee metrics related to retention, pay, services rendered, and marketing expenditures. These can help identify strengths and challenges of the agencies’ workforce structure.

The symposium was hosted by the National Direct Service Workforce Resource Center May 8-9, 2008 in Baltimore, MD with support from HHS and the Dept. of Labor.

This assessment is designed to assist organizations in evaluating their readiness to perform key functions of ADRCs. The online version provides users with immediate feedback and links to resources to help build needed capacity. Functional areas measured include: organization and governance; personnel and training; service delivery and operations; outreach and marketing; IT; partnerships; and evaluation and monitoring.

This is the third in a series of reports examining various state systems and processes directed to older adults to realize the goals of the Real Choice Systems Change grants. This report analyzes the results of twenty-four focus groups consisting of 240 participants representing older adults, caregivers to older adults, and HCBS providers. In general, the results indicate Illinois has a plethora of programs and services targeting older adults, but there are disparities between communities.

This study is intended to describe the nursing facility level of care determination processes in use by the states and to draw general conclusions from the data collected. When read in conjunction with the previous work in this area, this study will aid state staffs, CMS, and other parties interested in understanding how to improve post acute care assessment. The Appendix provide a brief description of data the state collects, the name of the assessment form, plus contact information.

The indicators provided in this fact sheet serve as a means for self-assessment by employment programs to help make strides toward facilitating competitive and sustainable employment choices for people with disabilities. This tool can also be used to identify both areas of strength to highlight in marketing and areas that need priority attention for improvement.
