Questions address contact information, income, employement, disability, insurance, benefits, and living arrangements. There is also space for referent information.

Following the Gulf Coast Hurricane disasters in 2005, the NASDDDS Emergency Response Preparedness Self Assessment Instrument was developed to assist state developmental disabilities officials evaluate the extent to which their existing preparedness plans address the unique characteristics and needs of people with intellectual and developmental disabilities during periods of emergency. This report was authored by PHI for the Health Field Collaborative (HFC) in Michigan to bring attention to its approach to improving retention of entry-level workers in health care and direct-care workers in the long-term care industry. The study explains how HFC created the Opportunity Partnership & Empowerment Network (OPEN) to address a critical local need. It outlines components of the program, reviews outcomes for the pilot phase, and identifies factors for sustaining it in the future.

This guide offers a quick reference on housing resources that can be used to create integrated, affordable, and accessible housing and basic information on how to use these resources. Including community strategies that increase the amount of integrated, affordable, and accessible housing units that usually fall into three general categories: development of housing units; access to existing housing units; and asset development.

This report reviews issues integral to the employment of people with disabilities. It has two broad aims: 1) to summarize existing knowledge regarding employment of people with disabilities; and 2) to present new information on the perspectives of employers, people with disabilities, and disability specialists on key barriers and facilitators of employment. The National Council on Disability offers recommendations to supplement best practices and existing public policies and initiatives.

Partnerships to Independence (PTI) was created by the Birmingham Independent Living Center in Birmingham, Alabama through a grant from CMS. This transition guide addresses nursing home outreach, support needs and skills, follow-up, and barriers. The purpose of PTI was to develop a model program that could be replicated in other CILs in Alabama and the US. ILC staff identified a number of basic resources and strategies that can be used to promote nursing home transition.

The MASS C-PASS Marketing & Outreach Subcommittee (M&O) created a Consumer Forum Series to gather information on barriers to self-direction and community integration from a community perspective. Aside from targeting elders, individuals with disabilities and their families, emphasis was placed on targeting traditionally underserved and un-served racial, ethnic, and linguistic minority groups and rural areas. This report is the narrative account and outcomes from nine consumer forums.

The fundamental structure to both of Oklahoma's Real Choice System Change grants is an infrastructure that supports a service delivery system that is accountable to Oklahoma long-term care stakeholders. Grant monies will be used to create and support an Oklahoma Partnerships consisting of consumers and advocates. This handout offers an introduction to the system and terms used in the grant partnership.

The Oklahoma Department of Human Services Aging Service Division presents this documentation of the principles and processes used in supporting its consumer advisory committee for Real Choice System Change grant and CDPASS grant activities. Building and documenting a shared vision of effective public policy funded by public dollars is a fundamental responsibility of grant recipients. This paper helps explain the stakeholder experience and considers ways to create a quality process.
