Maryland Access Point (ADRC) Marketing and Outreach Plan

This draft marketing plan from the Maryland Access Point (MAP), the ADRC program in Maryland, primarily addresses the marketing of the web-based information system in development. Link-Age Aging Resource Connection-Indiana’s ADRC Brochure, PSA and Website

Part of the national Aging and Disability Resource Center (ADRC) network, Link-Age is an evolving concept in Indiana and growing. The informational brochure provides consumers a way to discover their website and the video provides another way to get information. From the site you can either be connected to a local agency’s web site or search the database for resources.

Certified HOME Program Specialist Training Initiative

The Office of Affordable Housing Programs is now pleased to announce this new training initiative which will use a modified 'Building HOME' primer course as its foundation. Two and one-half days of training will be provided followed by a certification test. The training and testing will happen in various locations across the country and will focus on the HOME regulations. At this time only PJ or HUD staff with at least one year of direct HOME experience are permitted to register.

Sustaining Nursing Home Transition

The Nursing Home Transition Summit, convened by Rutgers Center for State Health Policy, was held September 28 through September 30, 2005. It brought together 39 participants from ten states, in addition to representatives from CMS, to discuss how to sustain nursing home transition programs. The New Jersey Community Choice Counseling program served as the summit centerpiece, with presentations by program staff and visits to several sites in the Central New Jersey area.

Diversion and Transition Services in the U.S. Promising Practices and Options for the Future

The report describes programs in various states whose primary purpose is nursing facility diversion or transition and identifies promising practices that have helped states divert or transition individuals from nursing facilities. Drawing upon those experiences, the report provides a variety of recommendations for states to consider in addressing the barriers and strengthening efforts to divert and transition individuals from nursing facilities.

Indiana\'s Efforts to Help Hoosiers Prevent Unwanted Nursing Home Residence

This State Policy in Practice brief describes Indiana’s efforts to develop a new, comprehensive program that helps people in Indiana (the Hoosiers) avoid nursing home placement if possible and desirable. It also describes pilot programs to transition those who choose to enter a nursing home, but then want to return to their homes. It is intended to help state policy and program leaders, advocates, and providers learn from their peers and colleagues across the states.

The State of 21st Century Long-Term Services and Supports: Financing and Systems Reform for Americans with Disabilities

Distressed Conditions in Developments for the Elderly and Persons with Disabilities and Strategies Used for Improvement

A new GAO report examined public housing developments occupied primarily by elderly persons and non-elderly persons with disabilities. Review the data on severely distressed housing and the ways in which such housing can be improved. This report examines the methods used to reduce the level of social distress and strategies to improve physical and social conditions.
