West Virginia Real Choice Community Outreach Material

For the last two years, WV Real Choice has worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. Attached are the program's community outreach materials such as brochures, flyers, postcards and previous and current conferences advertising materials.

Parent Leadership: Creating Partnerships During Health Care Visits

Family Voices of North Dakota Website and E-newsletter

Family Voices of North Dakota is a state chapter of the National Family Voices organization and provides health information and education. As part of the Family to Family grant, this chapter has begun to produce <i>Pass It on…Health Care Policy</i>, an e-newsletter that shares resources, information, and activities regarding care for children with special health care needs. Also visit their website for publications, a link directory, and other interactive resources.

Personal Assistance Direct Support Workforce Survey

The West Virginia C-PASS Grantees developed and designed this survey to determine the status of direct support workers providing home and community-based personal assistance services. The survey was distributed to agencies where workers provide services such as homemaker and personal care. The information gathered will be used by state policy makers to guide strategic planning and improve service delivery. The data will also help to implement and evaluate workforce development.

New Jersey Family Care - Website and Presentations

New Jersey Family Care is a federal and state-funded health insurance program which helps uninsured children receive affordable health coverage. The SPAN of NJ program has developed a number of presentations that explain their program, provide an overview of all medical insurance options, individual health plans and the other vital information. Presentation titles include: New Jersey Family Care, Medically Necessary Services, Individual Health Plan, EPSDT, and Medical Insurance.

The Medical Home Concept Presentations

Medical Home is an initiative funded by the MCH Bureau and HRSA and administered by SPAN of New Jersey as an approach to providing high quality, cost effective health services. The concept is for coordinated & respectful health care partnership among a child, his/her parents and the child’s Primary Care Provider. The presentations provide an overview of partnering.
