The Maine CHOICES CEO (Comprehensive Employment Opportunities) project seeks to develop and implement a comprehensive statewide plan to support more people with disabilities who work, or would like to work. The project will also improve information about, and access to, some key work incentives for Maine people with disabilities. The website features materials & resources, links to other work incentives resources and a description of the planning process.

The Hawaii Medicaid Infrastructure Grant, called Hire Abilities–Hawaii, is brand new this year but a dynamic website has already been unveiled. The site offers information about the project’s goals and strategies, advisory board and staff, and work incentive programs. It also serves as an important communication tool through the use of a public discussion board and on-line calendar. As an added feature, the site contains administrative tools to help the project team manage grant activities.

As part of the State of Wisconsin’s Quality Close to Home (QCTH) project, a contract was established with the University of Wisconsin – Madison’s Center for Health Systems Research and Analysis (CHSRA). This report describes the uses, methods, advantages, and disadvantages of four distinct sets of quality indicators, also called personal outcomes and performance outcomes. It also provides a overview, structural review and observations of the quality indicators.

The Ohio Department of Mental Retardation and Developmental Disabilities (ODMR/DD), a 2003 Real Choice Systems Change QA/QI in HCBS grantee, has begun to develop with initial stakeholder input, a Quality Framework to guide what data is collected. Ohio's QA/QI in HCBS grant application was written specifically to fund the design, development and implementation of all aspects of a quality information management system. The Children with Special Health Care Needs Program developed the outreach materials in October and November of 2004. You will find the radio scripts and sample radio ads for many of the outreach campaigns including: Clinging to Hope, Aphasia, Cleft Lip, PKU, Severe Burns, Heart Defects, Newborn Screenings, Basic 800 info spot, and the Covering Kids and Families Health Care Project. The program aired in both English and Spanish in cooperation with the Nevada Broadcasters Association.

The Wisconsin Department of Health & Family Services is designing an integrated state-local quality management system for the state’s HCBS long-term care programs. The Long Term Care Functional Screen (LTCFS) is a key element of their program, used for eligibility determination and rate settings. This paper explores ways to discover accuracy in the Level of Care determination process and describes an approach to LTC FS building on enhancements of current efforts and incorporating new activities.

As Florida creates new Aging and Disability Resource Center (ADRC) initiatives, the state has developed a number of Memorandums of Understandings (MOUs). A copy of the state, regional and local level agreements is attached. The purpose of the Memorandum of Agreement is to set forth the roles and responsibilities of each party in the development, implementation, operation and evaluation of the ADRC.

The ADRC in La Cross County, Wisconsin, is using this advertisement to inform the community about the ADRC program. Their Resource Center provides information and assistance by telephone 24 hours per day, including weekends and holidays.

The Minnesota ADRC utilizes the following two screening tools to assess the long term care needs of individuals and persons with developmental disabilities. Questions include general information, an assessment section, history and professional conclusions.

MinnesotaHelp.com is the state ADRC information network and they have shared a number of resources offering information on their Information and Assistance Datebase.
