Real Choice Systems Change Grants: Compendium Fifth Edition

The Compendium contains basic information about each of the Real Choice Systems Change Grantees. The Compendium will help you learn more about how these grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. Real Choice Systems Change Grantees will also find the Compendium useful to identify other Grantees with similar goals and activities. The fifth edition of the Compendium replaces all previous editions.

Streamlining Self-Assessment: Wisconsin

This worksheet helps ADRCs assess the hoops that consumers have to jump through to receive services. The goal of the worksheet is to aid ADRCs in planning for seamless entry into long term supports for consumers and serve as leaders in systems change. This tool can also be used for internal planning or with advisory or governing committees. Fill in the lines and boxes and see where to streamline and where there are barriers.

Maryland Access Point (ADRC) Advisory Board and Sub-committee Job Descriptions

The role of the Maryland Access Point (MAP) Advisory Board is to guide the design and operations of the Resource Center. MAP developed the attached job descriptions to assist board members in carrying out their roles efficiently. The board is composed of individuals representing all populations served by the program, including individuals who have a disability or a chronic condition and public, private, and government agencies that provide services to individuals served by the program.

The Washington Medicaid State Plan Personal Care Services Program

The Center for Personal Assistance Services has prepared this case study of the Washington state Medicaid Personal Care Services (PCS) program. This report first presents an overview of the states characteristics, and then reviews the Medicaid program and management of HCBS programs including the personal care in waivers. The primary focus is on the organization and management of the Medicaid PCS optional state plan program. The report highlights eight best practices.

ADRC Functional Staffing Model and Job Descriptions

The goal of the New Hampshire Resource Center project is to integrate the current information and assistance (ServiceLink) program with the state assessment and counseling program. This ADRC pilot program has shared their functional staffing model and job descriptions. In addition to being a visual illustration of staff resources, the documents also provide basic purpose, accountabilities and requirements for the Center Manager, Long Term Support Counselor, and Referral Specialist.

ADRC Stakeholder Satisfaction Report: A Comprehensive Assessment of Consumer and Board Member Satisfaction

This report summarizes the results of the Massachusetts qualitative consumer satisfaction surveys with both ADRC consumers and surveys of the Consumer Board and Community Partners Advisory Board. The report contains the survey tool, methodology, findings and recommendations to improve satisfaction among these key stakeholder groups as part of the ADRC project’s continuous quality improvement approach.

Promising Practices in HCBS: Minnesota-American Indian Tribe Providing Assessment and Case Management for Home and Community-Based Waiver Services

The State of Minnesota contracts with the White Earth Nation to provide functional eligibility determination and case management for Medicaid and state-funded home and community-based services for seniors. Tribal staff report their member are more likely to use the services when they can access them through familiar tribal health services. The federal government pays 100% of costs for Medicaid services provided by tribal organizations.
