Exploring the Potential Use of MDS Section Q1 Conference Call

Call hosted by the Community Living Exchange Collaborative at Rutgers Center for State Health Policy with CMS and grantees to explore the potential use of the MDS section Q1 to identify people in nursing homes who want to relocate. We were joined by a member of our National Advisory Committee, Bob Kafka. We invited Money Follows the Person grantees and Nursing Home Transition grantees.

CMS letter to State Medicaid Directors: Additional Examples and Clarification Money Follows the Person

On September 17, 2003, CMS sent a letter to state Medicaid directors addressing the principles of Money Follows the person. This letter is a follow up to the August 13, 2004 letter. Because the concept has been widely discussed in “Open Door” forums, this letter provides additional examples and clarification of the concept.

You Have a Choice! DVD in four formats: standard, with English subtitles, with Spanish subtitles, and audio described. The video is excellent for informing consumers about their options to leave a nursing home and to educate nursing home staff, family members and the general public about the option of transition. Partnership Development Activities: Comparative Analysis of FY 2001 Systems Change Grantees

This report provides a summary of the partnership development activities undertaken by the 52 Grantees that were awarded a Systems Change Grant in 2001. It describes the ways that Grantees involved consumers and public and private partners in the development of Grant applications and their plans for involving them in implementation activities. In addition, the report includes a more in-depth description of partnership involvement for nine Grantees (three from each Grant type).

Direct Service Workforce Activities of the Systems Change Grantees

Of the Real Choice Systems Change grantees awarded funding under CMS’s Fiscal Year (FY) 2001 solicitation, 20 have one or more initiatives to improve the recruitment and retention of direct service workers. This report focuses on the workforce initiatives of these 20 Grantees, with an in-depth look at 7 Grantees (AR [2 grants], KY, MT, NH, NC, VT, with whom RTI conducted site visits).

Sustaining Nursing Home Transition Programs: Covering Case Management Services Call Summary

A transcript and summary of the primary options for covering case management as discussed during a call on 11/10/03 with Mary Jean Duckett are provided here. The three options states may use to cover case management under Medicaid that are relevant to NFT were discussed. These included: 1915 (c) home and community based waivers; Targeted cased management services; and Administrative activity.
