Nursing Facility Transition

Beginning in 2001 and continuing in 2002 and 2003, technical assistance for the Real Choice Systems Change grantees has been provided by the Exchange Collaborative team (combined efforts of ILRU and Rutgers University/CSHP). These documents provide contact information for the TA providers and two charts indicating which Exchange Team is responsible for providing TA to the grantees. In 2002 & 2002, TA was divided according to states. In 2003, TA was distributed according to theme.

During this teleconference on 11/10/2003, Mary Jean Duckett from CMS presented answers to the grantee question: \"How do we continue working with people in nursing homes after our grant ends.\" Three options were discussed: Transition can be covered as a service under the Home and Community-based waivers; as a targeted case management service under the state plan or reimbursed as an administrative activity.

The National Council on Disability (NCD) assesses the nation's response to the United States Supreme Court's decision in Olmstead v. L.C., 527 U.S. 581 (1999) that the unjustified institutionalization of people with disabilities is a form of discrimination.

This article is entitled: \"Mentally Ill Adjusting to Community Settings\". The article describes the personal experiences of people with mental illness who leave institutions to live in the community.

These slides were presented at the 2nd Annual Training Institute for State Mental Health Olmstead Coordinators (9/30/2002 - 10/2/2002). They provide a brief overview of the FY01 and FY02 Systems Change Grants, including charts depicting which states are focusing their efforts in areas such as housing, mental health, nursing facility transition, etc.

The Compendium contains basic information about each of the Systems Change Grantees. Each section is alphabetical by state. Information for each state includes: the name of the grantee organization, the title of the grant, the type of grant, the amount awarded and fiscal year awarded, the primary contacts for each grant, the target populations to be served under the grant, the primary goals and activities of each grant project, and a brief description of the grant activities.

This Community Living Brief explores the transition to an independent, integrated living situation. This article serves as a guide to assist the individual when considering moving back into the community. The article provides information on finding an advocate and also provides a transition checklist to help facilitate the moving process.

This Working Paper explores key policy issues arising from the United States Supreme Court’s decision in Olmstead v. L.C. Specifically, this paper examines how states have responded to the Court’s suggestion regarding the development of comprehensive, effectively working plans for placing qualified persons with disabilities in less restrictive settings as a way of satisfying the ADA’s “reasonable modifications” requirement.

This report is focused on the status of Family Care implementation in WI. It contains background information on the Family Care program, as well as evaluative information on the development of each pilot project. The report also contains a glossary of terms and a research methodology section that demonstrates the contacts made by The Lewin Group in the five pilot counties—Fond du Lac, La Crosse, Milwaukee, Portage and Richland—in order to assess the current status of the Family Care program.

This report describes Community Living Excahnge Collaborative's technical assistance needs assessment of the 52 Real Choice Systems Change grantees operating in 37 states and one territory (Guam). The report also describes the findings of the needs assessment.
