This webinar will discuss the benefits and limitations of advance care planning as a person-centered approach to care. Advance care planning and health care directives are written instructions that explain the type of medical care you want in the event that you are not able to communicate your preferences. Older adults, caregivers, legal service professionals, social workers, service coordinators, bank and credit union staff, faith leaders, librarians, and law enforcement officers are all welcome to attend.
