Resources for Integrated Care (RIC) is hosting a conversation about promoting self-management for individuals dually eligible for Medicaid and Medicare in wellness planning. The Institute of Medicine defines self-management as “the tasks that individuals must undertake to live well with one or more chronic conditions”. These tasks can include diet, exercise, socializing, and other daily tasks. Judith Cook, PhD- who consults with federal agencies such as the National Institutes of Health and the Veteran’s Administration- will facilitate the conversation.
RIC is encouraging those interested to submit questions before the discussion via email (RIC@Lewin.com), direct message on Twitter (@Integrate_care), or at the time of registration.
