This guide was created to help serve as a resource for seniors and caregivers when you making decisions about transitions. The guide can be used as a tool to help think about many important aspects of transitions from hospitals, nursing homes, assisted living facilities, and community based living. The guide was written in collaboration between numerous organizations in Maine.

This curriculum to train personal assistants has two elements: a competency or skill development component and a didactic or knowledge development component. The skill development components include Connecting, Coaching, Collaboration, and Managing Crises. The knowledge development component is a web-bases curriculum entitled Personal Assistance Services for Individuals with Serious Psychiatric Disabilities.

The IHSS handbooks were the product of many people’s efforts. The topics covered were determined by input from the IHSS Enhancement Initiative Task Force, focus groups of consumers, providers, social workers, and public authority staff in three California counties. The guide includes topics such as assessment, public authority, hiring, practical details like timesheets and benefits, communication and organization skills. Publications are also available in Armenian, Chinese, Russian, and Spanish.

What is Supported Employment? The following presentations, brochures and manuals explain the ways that supported employment helps consumers find and maintain meaningful jobs in the community. The Alaska MIG grantees have shared two presentations and brochure on the principles of supported employment. The manual is designed as a guide for employment specialist.

This paper presents the perspective that options counseling is a pivotal function of Aging and Disability Resource Centers (ADRCs) and its essential elements are counseling and decision support. The diverse array of definitions currently in use runs the risk of diminishing the importance of options counseling, which is a key function of a comprehensive long-term care system.

This manual talks about the steps that need to be taken in order to hire and manage a personal care attendant. Topics that are discussed in the manual include knowing what you need and want, the hiring process, tax considerations, what being an employer involves, stress management, communication skills, plus additional resources.

The Utah Real Choice grant produced “The Caregiver Guide” and they have shared several modules. Topics covered include: Community Supports to Stay at Home, Caring for a family member with mental illness, Hiring and managing personal assistants, Assistive technology, Creating an accessible home, and Caring for a child with special needs. Modules are also available in Spanish.

The Ohio Access Success project has shared various forms associated with their project aimed at nursing facility transition. The forms aid the planning and implementation of the transition process. Forms include Consumer Assessment, Referral Form, Relocation Plan and a Role Matrix.

The Consumer-Directed Supports Toolbox is a set of materials developed under the Community-Integrated Personal Assistance Services and Supports (CPASS) grant awarded to North Carolina in 2002. It was developed to support the implementation of consumer-directed options in North Carolina. The “toolbox” consists of 3 powerpoint presentations, a set of exercises to be used in conjunction with the powerpoints, a consumer-directed supports manual, brochures and other visual aids.

This series of worksheets provides examples of what the Michigan Nursing Facility Transition grant used in planning the transition from the nursing facility to the community.
