Toolkit

The Toolkit was designed to help hospitals and community partners address the needs of people with chronic conditions by improving self-management skills. This toolkit provides information on the benefits of starting and sustaining a Chronic Disease Self-Management Program (CDSMP), helpful sources and materials implementing the CDSMP.

When assessing the needs of persons with dementia and caregivers in the community, it is important to understand the number of persons affected and their unique needs for services and programs. This toolkit complements the issue brief titled Making the Long-Term Services and Supports System Work for People With Dementia and Their Caregivers by identifying resources that states and communities can use to design initiatives to ensure that programs are dementia-capable.

The Exchange website offers busy health professionals and researchers a variety of opportunities to share, learn about, and ultimately adopt evidence-based innovations and tools suitable for a range of health care settings and populations. The site has the following components: Searchable innovations; Searchable QualityTools; Learning opportunities and Networking opportunities.

The Administration on Aging (AoA) has a new page on its website that highlights program and funding opportunities within the Affordable Care Act that relate to the mission and vision of AoA and the Aging Network. Topics covered include Medicare and Medicaid, prevention, and webinars and technical assistance resources.

The U.S. Department of Labor's Office of Disability Employment Policy’s (ODEP) new toolkit includes discussion guides and answers to frequently-asked-questions for people looking for jobs, employers, community employment agencies and others. Integrated employment places both youth and adults in regular jobs in their communities, along with any needed supports, so they can earn a decent wage. In an integrated employment setting, most of the employees are not persons with disabilities.

This site is a one stop resource to help expand integrated supportive housing approaches for people with disabilities. It highlights innovative new options within the U.S. Department of Housing & Urban Development (HUD) Section 811 Supportive Housing for Persons with Disabilities program, & provides information & resources targeted to states & other supportive housing stakeholders as they face new challenges & opportunities for ensuring community living for non elderly people with disabilities.

These Guiding Principles were developed to help to preserve the rights of individuals under participant direction, no matter what form or model, if and when workers choose to form a union. This Toolkit provides information for communities interested in working together to develop a one-call or one-click service for transportation to fit their own circumstances. On November 9th, the Federal Transit Administration announced $34.6 million would be awarded to 55 communities for resource centers. Review the website for practical guides and tools on implementation. Additionally there are community profiles and webinars to provide best practice examples.

The National Post-School Outcomes Center (NPSO) developed a toolkit to assist States and local education agencies, their stakeholders, and partners in using post-school outcome data to improve programs for youth with disabilities. The Data Use Toolkit consists of two components: preparing and sharing post-school outcomes data and examining local post-school data. Explore this guide for facilitators to learn more.

How is California helping youths with disabilities to connect with needed supports and services as they transition to work or postsecondary education? Explore several presentations and toolkits put out by the California Health Incentives Improvement Project (CHIIP) that aim to facilitate the transition process for these youth. May be replicable by other states seeking to assist similar populations.
