This Toolkit provides information for communities interested in working together to develop a one-call or one-click service for transportation to fit their own circumstances. On November 9th, the Federal Transit Administration announced $34.6 million would be awarded to 55 communities for resource centers. Review the website for practical guides and tools on implementation. Additionally there are community profiles and webinars to provide best practice examples.

The National Post-School Outcomes Center (NPSO) developed a toolkit to assist States and local education agencies, their stakeholders, and partners in using post-school outcome data to improve programs for youth with disabilities. The Data Use Toolkit consists of two components: preparing and sharing post-school outcomes data and examining local post-school data. Explore this guide for facilitators to learn more.

Explore several presentations and toolkits put out by the California Health Incentives Improvement Project (CHIIP) that aim to facilitate the transition process for these youth. May be replicable by other states seeking to assist similar populations.

Oregon’s CLP Options Counseling Protocol provides guidance for receiving, contacting, meeting and communicating with referrals for options counseling through the Community Living Program (CLP) grant. This could serve as a template for other organizations seeking to develop Options Counseling procedures.

PSWH is a person-centered planning process that looks comprehensively at a person’s health life-style, focuses on a person’s strengths, interests and natural supports, stresses creating new health life-style habits and disciplines, and provides peer support delivered by peer specialists to promote self-directed whole health. States are developing comprehensive APS systems that include assessments, policies and procedures to assist staff in performing intakes, investigations, and case planning by providing a consistent approach to obtaining and evaluating information. The SDM® system is intended to promote the safety of vulnerable adults, identify and address their needs, decrease the incidence of self-neglect and maltreatment, enhance service delivery, and provide data for program administration.

These informal, person centered thinking tools, created by The Learning Community for Person Centered Practices and the Inclusion Movement, were developed by cancer survivors to help individuals find empowerment or a sense of control in their experience. The website offers a number of on-line tools and worksheets that could be useful to individuals with disabilities and those who support them. Workshops, coffee chats and consultation sessions are offered as a way to help people use the tools.

Use this health literacy toolkit to make written material in printed formats easier for people to understand and use. Though the main focus is on material intended for use by individuals eligible or enrolled in programs, the guidelines also apply to material created for people who serve or assist them. The toolkit has 11 parts, including guidelines for writing and design, methods for testing with readers, and guidelines for translation.

DB101 provides online tools, resources and information on health coverage, work and benefits for people with disabilities and those supporting them. Four states (California, Minnesota, Michigan and New Jersey) have their own sites, which include benefits calculators to help individuals understand how a job may affect total income and health coverage, or whether they may qualify for specific state programs. The resource is a product of the Employment and Disability Benefits Initiative.
