Long-Term Support Direct Care Arrangements in Wisconsin Counties: Survey Results, 2004

This report summarizes information from the WI Department of Health and Family Services, Bureau of Aging and Long Term Care Resources (DHFS-BALTCR) Nursing Home Transition Real Choice Systems Change grant. A survey colleting information is an important initial step to assist county and state-level program and policy planners. This report provides an overview of the ways in which counties purchase direct care and support services for elderly people and people with disabilities.

Tennessee Personal Assistant Supports & Services: Self-Assessment Tool and User\'s Guide

The ARC of Tennessee developed this Personal Assistance Supports and Services Guide to provide people with disabilities the information & tools needed to hire and manage a Personal Assistant. Consumers first complete the self-assessment tool to determining needs. This entry also includes the user's guide and evaluation feedback form.

Pennsylvania Community Choice Customer Satisfaction Surveys

There are three customer satisfaction surveys from the Pennslyvania Community Choice program included here. There are two surveys for consumers found to be eligible for HCBS services: one for those who enrolled and one for those who did not. The third survey was intended for consumers who were found to be ineligible for HCBS services.

More Can Be Done: Involvement of Older Consumers in the Design, Implementation and Oversight of Home and Community Based Services

This paper explores the role of older participants in planning, developing and overseeing home and community based service (HCBS) programs. It seeks to help State Units on Aging and others in the aging network identify concrete steps they can take to support the role of these older individuals as public policy advocates. The report addresses the extent of older HCBS participants' current involvement, the barriers to success, and the strategies being used to attain participation.

Vermont Work Incentives Initiative Website

The Vermont Work Incentives Initiative is comprised of several interconnected grant projects that fund direct services to clients, research, and systems change efforts focused on improving employment outcomes and independence for people with disabilities. The web site prvides a wide array of resources on several topics, including benefits counseling, medicaid buy-in program, personal assistance services and other employment supports.
