Individual Support Plan System

Developed by Oregon’s Office of Home & Community these tools include consideration of 43 areas of risk and illustrates the state’s risk tracking documentation in Q&A format. They also provide instruction to staff with loops back to participant preference and guidance to protocols to address identified risks. Forms included: ISP Addendum, Financial & Individual Support Plan, Personal Focus Worksheet, Risk Training, Protocol for Aspiration, Constipation, Dehydration, Seizure and Signature Sheet.

Plan of Protective Oversight

Developed by the New York Department of Health this tool is used for participants with traumatic brain injury. The tool identifies risk factors and the services needed to mitigate them, and assigns specific persons who will be responsible for providing the necessary service and oversight. Addressed in the plan: money management, medication management, kitchen safety, back-up staffing for unscheduled staff absences. Plans are revised at least every six months and must be signed.

Handbook for Transition Navigators

This handbook, drafted by Jane Schiele and edited and revised by Laura Hershey (HCPF Systems Change Section), is designed to help other advocates or agencies to inform many more individuals about their right to live in the community, and to assist those who desire a transition back to their community. The report defines transition and offers the nuts and bolts of the program and lastly overs tips for moving day and beyond.

After Leaving the Nursing Home Form

This survey was developed by the Colorado Facility Transition grant to evaluate the transition process of individuals across Colorado. Results from this survey were used to develop and implement changes in the transition process to better serve the population. The form is for people who have moved out of a nursing home in Colorado within the last two years. How you fill in this form is important because the answers help other people who will move from a nursing home soon.

Evaluation Report on The Institute on Disability/UCED Educational Series for Legislators

This report compiles both written and verbal evaluations of twenty New Hampshire legislators who attended the Supporting Older Adults and Individuals with Disabilities in Your Community: An Educational Series for Legislators. The series was developed as part of the Real Choice Systems Change initiative in New Hampshire. The purpose of this evaluation was to review what aspects of the Educational Series were beneficial and to recommend improvements for future sessions.

The State of the States in Family Caregiver Support: A 50-State Study

This report focuses attention on a growing issue facing the nation: how to support & sustain those who are the cornerstone of our LTC system—the dedicated families & informal caregivers. This national study is the first to examine publicly funded caregiver support programs throughout all 50 states. The report identifies key findings, describes state approaches to providing caregiver support services, offers state-by-state profiles, pinpoints challenges, & expands recognition of caregiver needs.

Technical Assistance to Real Choice Consumer Task Forces-Website

The Centers on Medicare and Medicaid Services awarded a three year contract to the Topeka Independent Living Resource Center (TILRC) to provide technical assistance to the Consumer Task Forces Real Choice system change grant projects. In doing this, CMS highlighted the contributions that consumers can and must make to the success of the grants and to increasing access to community living services more generally. This website offers a list of grant projects, tip sheets and survey tools.

South Carolina Access Plus Survey Tools

The South Carolina ADRC also known as South Carolina Access Plus has developed a number of checklists and surveys to evaluate a variety of program outcomes. The Access Training is an introduction to the program. The Customer Service Training assesses staff opinion on customer service training sessions. The Level Outcome Checklist assesses staff opinion on how well the program is achieving systems-level outcomes. The Progress Checklist assesses staff opinion on collaborating with stakeholders.

Louisianaanswers: The Louisiana ADRC Website, Marketing Materials and Evaluation

The following documents provide an overview of the Louisiana ADRC called the Louisiana Aging and Disability Information Station. The first document outlines how advisory board members can market the program including the Louisiana ADRC Web site, www.LouisianaAnswers.com. There is also a report that provides a step by step narrative of the approach to marketing and lastly the status report on the ADRC program covers the period from August 1, 2004 to September 30, 2004.
