Consent Form and Focus Group Questions for the Granite State Employment Project Consumer Forum

The Monadnock Center for Successful Transitions issued consent forms to eligible consumers to complete in order to participate in the Granite State Employment Project Consumer Forum. The purpose of the forum is to study issues regarding employment for individuals with disabilities in the Monadnock Region. The information gathered will be used in the strategic planning process for the Granite State Employment Project, the New Hampshire MIG. Also included is a list of the focus group questions.

In Support of Oregon Children with Developmental Disabilities

The purpose of this study was to identify how DHS can use Medicaid and other resources more broadly and effectively to meet the family support and respite needs of Oregon families of children with developmental disabilities. Assess what SPD might do to improve its response to families through Family Support systems. This final report documents the activities, findings and recommendations associated with the study.

Consumer Survey for Use with Recipients of In-Home Services

The Oregon Department of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with HSRI to assist in improving the processes for assuring quality in SPD in-home waiver services. Drawing from HSRI's experience in quality assurance, SPD identified a number of states which have been particularly successful in conducting consumer surveys. At the same time, the project team explored in greater detail what was currently happening in Oregon around consumer surveys.

Community Participation of Persons With Disabilities In Connecticut

The Real Choice System Change project at the University of Connecticut’s Center for Disabilities administered a Community Inclusion Assessment to determine the level and quality of inclusion. The papers in this series are based on a sample of 250 individuals with disabilities. Briefing Paper #3 explores a gap between the level of involvement and feelings about involvement. Findings suggest that assuring the opportunity of participation is only one step in making communities more inclusive.

Ticket to Work Project Survey: Alabama Employs Ability

The Alabama Department of Rehabilitation Services (ADRS) Medicaid Infrastructure Grant is working to develop health care supports that help people with disabilities become - and remain - employed. One of the key objectives focuses on students ages 18-22 in transition who have severe disabilities. The survey gathered information to inform and guide implementation and to identify areas for resource mapping in the demonstration sites.

Quality Improvement System Project Survey - Arizona

The Arizona Consumer-Driven Quality Improvement System Project developed two survey instrument to gather information regarding the attendant care services that consumers receive from the Division of Developmental Disabilities. They have also shared their EAzI(Empowering Arizona’s Individuals with Developmental Disabilities) Quality Traits which were created to assist in developing a survey instrument specific to the service quality needs of consumers who use Home and Community Based Services.

Person Centered Planning Groups

As part of the Georgia QA/QI Real Choice Systems Change grant, an evaluation of the Person-Centered Planning (PCP) program was conducted. Part of this includes the outline for planning and creating PCP groups, and a list of resources for effective use of PCP. Also included is the final survey given to PCP participants and information on sustaining person-centered planning.

A Survey of North Dakota Consumers of Continuum of Care Services

This report summarizes the research findings of a 2006 questionnaire disseminated to gather information about choice and access to continuum of care services. The survey gained information from consumers regarding services they are using, services needed, and barriers encountered. Data was gathered regarding how consumers learn about available services and suggestions to guide the development of a single point of entry (SPE) system, also called an Aging and Disability Resource Center (ADRC).

A Report of Questionnaires Administered to North Dakota Hospital Discharge Planners

This report describes the hospital discharge planner (HDP) survey conducted in the beginning of 2006. The HDP questionnaires were disseminated to gather information about the awareness of choice and access to continuum of care services for the elderly and people with disabilities as well as to gather ideas about how to optimize choice and improve access to these services. Review the results and read about the barriers faced by discharge planners.

Elder Choice: How do older people make long-term care choices?

This paper reports the outcome of focus groups, interviews and written surveys conducted in 2003 by a consultant working for the Northern Area Agency on Aging in Wisconsin under a contract with the Department of Health and Family Services. Groups of elders, family members and long-term care professionals were asked to address the question of how, when and why older people make the decisions they do about long-term care.
