Disability History Week-Surveys - California

How can organizations evaluate their youth oriented programs? One way California is doing this is through surveys given to attendees of the Youth Organizing Disabled and Proud Seminar. Attached are sample letters, surveys and permission forms needed to survey attendees. See also, "Disability History Campaign-Presentation and Resources" for more information about the seminar and "Disability History Week-Activities" for sample training activities.

Consumer, Family & Youth Conference Survey – Hawaii

What leads you to want to go back to work? This is just one of many questions asked at Consumer, Family & Youth Conference, where Hawaii introduced its Benefits Information Network in 2009. Designed for the panelist discussion, this series of questions and introduction to the Benefits Information Network, emphasizes the importance of work incentives information available to the public. This document could be used by other states interested in starting a MIG grant program.

Disability History Week–Presentation and Resources - California

How are states empowering youth with disabilities? Iowa Aging and Disability Resource Center Pilot Sites - Evaluation Report

Review an evaluation of the Johnson County and Linn County Aging and Disability Resource Center (ADRC) pilot sites. The evaluation assesses the process and outcomes of establishing a standardized entry point to long-term care supports and services, with emphasis on the role of the Options Counselor (OC) at both sites. Copies of surveys and interview techniques used are included.

Client Self-Reporting of Employment Supports – Nebraska

What do Nebraska’s Medicaid Insurance for Workers with Disabilities (MIWD) clients think about the employment supports offered them through the MIG funded program? In December, 2007 current clients were asked to participate in a telephone survey, which was a follow-up to a more extensive evaluation finalized in April 2007. The methodology, goal and summary of the findings are included here and could be helpful to other states looking to improve the effectiveness of their employment supports.

2007 National Home and Hospice Care Medication Data

Link to a public-use data file and to supplementary files containing information about the data collection process and resources to help analyze the numbers. The technical notes file contains information on how the medication data were collected and processed. The data dictionary gives the layout of the 534 variables in this file. The other files provide information to help users search for medications by brand and generic names, and analyze the medication data in the file.

Employer and Employment Questionnaires - New Hampshire

As part of its MIG grant, New Hampshire has been working to address issues affecting the employment of people with disabilities. One way this was done was by creating questionnaires for organizations to fill out themselves as well as to give employers. Two examples of these are provided, one to assess how employment-focused an organization is, and one to assess the needs of businesses and employers. These forms can be used as templates for organizations in other states.

Employing People with Disabilities – Montana

How can employers attract and retain employees with disabilities? This and other issues having to do with disabled workers are addressed in a brochure sent to Montana employers. This tool includes a self assessment questioner about workplace disability practices, a letter from the governor, a myths and facts section, and resources for more information. While this is meant for Montana residents, this could serve as a template for other states interested in employing more people with disabilities.
