Youth Leadership Toolkit: A Guidebook

The purpose of this toolkit (consisting of a DVD and guidebook) is to illustrate the importance of the perspectives that youth and young adults bring and to provide information and tools that help to effectively include them in addressing the challenges of transitioning to adulthood and increased independence. The DVD provides human stories regarding key components of living independently in the community. The Guidebook provides discussion questions by topic for each of the target audiences.

The Green House: A New Model of Care – Audio

The narrator interviews proponents, staff, and consumers of the Green House model. During visits to two Green Houses for people with Alzheimer’s in Texas, she discusses costs, outcomes, terminology, and guiding principles. According to the interviews, outcomes such as consumer satisfaction and well-being and employee retention and interaction are better than in institutional settings. The 11 minute audio file is part of the “New Directions in Health Care” podcast series.

Variation Analysis by Location and Understanding Efficiency (VALUE) Project

These resources come from a CMS-funded pilot program where four participating quality improvement organizations (QIOs) test interventions designed to understand and change health care cost and quality patterns that differ between regions. The CARE tools ensure quality maintenance during health care transitions by assessing and recording key data. The archived weekly learning sessions cover a variety of topics related to transitional care and include handouts and audio recordings.
