The purpose of this toolkit (consisting of a DVD and guidebook) is to illustrate the importance of the perspectives that youth and young adults bring and to provide information and tools that help to effectively include them in addressing the challenges of transitioning to adulthood and increased independence. The DVD provides human stories regarding key components of living independently in the community. The Guidebook provides discussion questions by topic for each of the target audiences.

In two short videos, Lana and Janet, participants in the Missouri MFP program, share their experiences moving out of nursing homes. They also discuss some program details and encourage others to sign up. The videos were produced by the Disability Resource Association to promote MFP.

Medicaid covers health and long-term care services for many millions of low-income Americans. Conversations were already underway at this event concerning the role of Medicaid as a vehicle for economic stimulus, and about health coverage expansion proposals, with particular discussion around the challenges facing states implementing the program in the recent economic downturn.

Program administrators, advocates, and researchers advise the Senate on ways to improve wellbeing and cost outcomes of long-term care by increasing access to HCBS. They speak on national demographics, successful HCBS programs of the past and present, state programs in Wisconsin and Florida, integrating acute and long-term care for dual eligibles, and program finance models. The narrator interviews proponents, staff, and consumers of the Green House model. During visits to two Green Houses for people with Alzheimer’s in Texas, she discusses costs, outcomes, terminology, and guiding principles. According to the interviews, outcomes such as consumer satisfaction and well-being and employee retention and interaction are better than in institutional settings. The 11 minute audio file is part of the “New Directions in Health Care” podcast series.

Watch a series of short, simple and direct videos meant to educate and train family caregivers. Videos cover a range of topics from infection control to paperwork. A “Beyond the Video” section offers video summaries and tips for applying lessons to each family's situation.

Watch employers attest to how their business’ needs are met by skills that workers with developmental disabilities have brought to the workplace. The five firms included in the video found that those skills are crucial as they convert to computerized record systems in order to increase efficiency and drive down record keeping costs.

Watch Danielle, Alexander, Valeska, and Santara share strategies they used as students with disabilities to successfully graduate from college and get jobs. They discuss their struggles with self-reporting their physical or mental disabilities and negotiating accommodations in school and at work. DVDs are available upon request.

These resources come from a CMS-funded pilot program where four participating quality improvement organizations (QIOs) test interventions designed to understand and change health care cost and quality patterns that differ between regions. The CARE tools ensure quality maintenance during health care transitions by assessing and recording key data. The archived weekly learning sessions cover a variety of topics related to transitional care and include handouts and audio recordings.

Watch these videos to hear first-hand from direct-care workers about the services they provide. They share their thoughts on why they do this work, how they are viewed by families, friends, and service recipients, and how they deal with challenges such as discrimination and disrespect.
