The Department of Mental Retardation (DMR) has established a system of reporting and monitoring incidents that occur with individuals served by the department in order to identify, manage and reduce overall risk for the individual. This procedure delineates a standardized process for reporting, documentation and follow-up of reportable incidents as defined, involving individuals served by the department. This reporting system helps implement individual plans and assists in quality oversight.

How are ADRCs and other non-profits using mobile technology? For staff who spend time traveling to clients homes and providing services in a non-office setting, portable information is a must. This issue brief provides some basic information about the most common forms of mobile technology and some issues to consider before adopting portable equipment as a service delivery tool. Also included are the ways information is accessed, collecting client data plus the approaches used by other states.

The Bangor Eastern Agency on Aging ADRC project has shared a number of resources involving the Intake and Referral Process. The first is their policies which provide staff guidelines for consistency. The second resource is their intake questions to help record and collect information. Lastly, they have shared their feedback referral process and how to make this simple and accessible by the consumers.

The report describes programs in various states whose primary purpose is nursing facility diversion or transition and identifies promising practices that have helped states divert or transition individuals from nursing facilities. Drawing upon those experiences, the report provides a variety of recommendations for states to consider in addressing the barriers and strengthening efforts to divert and transition individuals from nursing facilities.

This worksheet helps ADRCs assess the hoops that consumers have to jump through to receive services. The goal of the worksheet is to aid ADRCs in planning for seamless entry into long term supports for consumers and serve as leaders in systems change. This tool can also be used for internal planning or with advisory or governing committees. Fill in the lines and boxes and see where to streamline and where there are barriers.

The attached assessment tools are used statewide throughout the Aging Network to collect information to streamline the eligibility determination referral process. Individuals with a high priority based on the screening are referred for a comprehensive assessment prior to care plan development and enrollment for services.

The Center for Personal Assistance Services has prepared this case study of the Washington state Medicaid Personal Care Services (PCS) program. This report first presents an overview of the states characteristics, and then reviews the Medicaid program and management of HCBS programs including the personal care in waivers. The primary focus is on the organization and management of the Medicaid PCS optional state plan program. The report highlights eight best practices.

The Real Choice System Change project at the University of Connecticut’s Center for Disabilities administered a Community Inclusion Assessment to determine the level and quality of inclusion in Connecticut communities. The papers in this series are based on a (non-random) sample of 250 individuals with disabilities who completed an in-depth survey. Real Choice Briefing Paper #2 explores employment issues.

As turnover among direct care workers continues at very high rates, providers need to implement solutions that work and employee surveys can be an important tool in understanding staff satisfaction. This guide was developed to help organizations improve their use of measurement tools to understand direct care workforce problems and to inform their solutions. LTC providers and researchers can use the 12 instrument measures to assess monitor and improve the work environment.

This toolbox was developed for nursing facility transition (NFT) and Money Follows the Person (MFP) grantees to provide materials to support the design, development and operation of nursing facility programs. This toolbox includes over 30 resources to assist states in these efforts. Resources include existing materials or tools, CMS guidance and report developed by the Rutgers/NASHP team and others.
