Incident Reporting Forms and Policies: Connecticut

The Department of Mental Retardation (DMR) has established a system of reporting and monitoring incidents that occur with individuals served by the department in order to identify, manage and reduce overall risk for the individual. This procedure delineates a standardized process for reporting, documentation and follow-up of reportable incidents as defined, involving individuals served by the department. This reporting system helps implement individual plans and assists in quality oversight.

Diversion and Transition Services in the U.S. Promising Practices and Options for the Future

The report describes programs in various states whose primary purpose is nursing facility diversion or transition and identifies promising practices that have helped states divert or transition individuals from nursing facilities. Drawing upon those experiences, the report provides a variety of recommendations for states to consider in addressing the barriers and strengthening efforts to divert and transition individuals from nursing facilities.

Streamlining Self-Assessment: Wisconsin

Prioritization Assessment Instrument and Form: Florida

The attached assessment tools are used statewide throughout the Aging Network to collect information to streamline the eligibility determination referral process. Individuals with a high priority based on the screening are referred for a comprehensive assessment prior to care plan development and enrollment for services.

The Washington Medicaid State Plan Personal Care Services Program

The Center for Personal Assistance Services has prepared this case study of the Washington state Medicaid Personal Care Services (PCS) program. This report first presents an overview of the states characteristics, and then reviews the Medicaid program and management of HCBS programs including the personal care in waivers. Employment Of Persons With Disabilities in Connecticut

The Real Choice System Change project at the University of Connecticut’s Center for Disabilities administered a Community Inclusion Assessment to determine the level and quality of inclusion in Connecticut communities.
