Final Week to Apply: Making Care Primary (MCP) Model

The Centers for Medicare & Medicaid Services (CMS) have issued a notice that the deadline for applying to participate in the Making Care Primary (MCP) Model is next week. The MCP application is non-binding and should take no more than 15 minutes to complete. Eligibility requirements and additional model details can be found in the Request for Applications (RFA). To support interested applicants in their decision-making, CMS has published an Example Revenue Factsheet which compares potential revenue under Medicare fee-for-service to potential revenue under MCP. Application Period is Open for NCAPPS Technical Assistance

The application for the next round of NCAPPS Technical Assistance is now available. Technical Assistance will be provided to up to ten States, Tribes, or Territories for up to 100 hours per year for up to two years. Technical assistance will help advance person-centered thinking, planning and practice to support people with disabilities and older adults with long-term service and support needs. Applications are due by July 30, 2021.

Guidance to States using 1115 Demonstrations or 1915(b) Waivers for Managed Long Term Services and Supports Programs

CMS has released two important documents that outline essential elements of MLTSS programs that are in line with current best practices. The first document summarizes these essential elements and the second provides guidance for incorporating these essential elements into MLTSS programs operating under section 1115 or 1915(b) authorities. These standards will be used when CMS reviews states’ design, implementation and operation of MLTSS programs for approval and ongoing quality assurance.

Health Home Information Resource Center

The Affordable Care Act authorized the Medicaid Health Home State Plan Option, which allows states to design health homes to provide comprehensive care coordination for Medicaid beneficiaries with chronic conditions. States will receive enhanced federal funding to support the rollout of this new integrated model of care. This link directs to the Health Home Resource Center that provides technical assistance services for states as they submit their proposals for these health homes to CMS.

Financial Stability Initiative: Indiana

The Indiana Medicaid Infrastructure Grant’s (MIG) Asset Development/Financial Literacy Project is developing a Financial Stability Pilot to engage people with disabilities in financial literacy and asset development programs. The two main goals of the program are to increase the number of individuals with disabilities who are able to access and benefit from asset development and increase collaboration amongst the asset building and disability fields. Alzheimer’s Disease Demonstration Grants to States (ADDGS) Projects – AoA Demonstration Grants

AoA announced the availability of $10,900,000 for two ADDGS Program Announcements: "Innovation Grants to Better Serve People with ADRD" and "Evidence-Based Intervention Translation Grants to Serve People with People with ADRD and their Caregivers." The goal is demonstrate how the Aging Services Network can effectively implement evidence-based service interventions that help people with ADRD to remain at home and in the community. Closing dates for applications are August 11th, 2008.

Nursing Home Diversion Modernization Grant including a Special Funding Opportunity to Serve Veterans: Grant Announcement

AoA announced $13 million to support state efforts to help individuals with long-term care needs to avoid unnecessary nursing home placement. This includes $10 Million for an AoA Nursing Home Diversion Modernization Grants Program and a special $3 Million funding opportunity made available by the Veterans Health Administration. Only State Units on Aging are eligible to apply, but must involve AAAs and community-based organizations in projects. The application deadline is August 14, 2008.

Minnesota Comprehensive Employment Opportunities - Medicaid Infrastructure Grant: Strategic Plan & Funding Application

Developed by the Comprehensive Employment Opportunities – Medicaid Infrastructure grant, a strategic plan project was initiated to increase the competitive employment of people with disabilities in Minnesota. The report, based on data obtained through individual interviews and focus groups in 2005, is included. Additionally, the Continuation of Funding Application for 2006 is available.

Nursing Home Diversion Modernization Grants

AoA announced the availability of $5 million for these grants which provide an opportunity for States to modernize their existing efforts by adding flexible service options to help individuals who are not eligible for Medicaid to avoid nursing home placement and spend down to Medicaid, and to remain in their homes and communities. The applicant teleconference is scheduled for July 11 at 3pm. Submission of letter of intent is for July 20, 2007 and the deadline is August 24, 2007.

FY2007 Real Choice Systems Change (RCSC) Grants Solicitation

This application offers a total of approximately $13 million in Real Choice Systems Change (RCSC) grants to qualifying States in two grant categories: State Profile Tool (SPT): Assessing a State's Long-Term Care System & Person-Centered Planning Implementation Grant. Persons with mental illnesses are noted as target populations for both grant categories. Applications are due by July 27, 2007.
