Current Living Situation and Service Needs of Former Nursing Home Residents: An Evaluation of New Jerseys Nursing Home Transition Program

As part of its evaluation of the New Jersey Senior Initiatives Community Choice Counseling Program, the Center for State Health Policy at Rutgers conducted an evaluation of former nursing home residents who were discharged through the Community Choice Counseling Program. This report addresses clients' current living situation, use of informal and formal assistance, service deficiencies, satisfaction, and quality of life.

Finding Common Ground: Creating Inclusive Communities Teleconference & Report

Sponsored by the Community Living Exchange Collaborative at ILRU this teleconference featured Real Choice Systems Change grantees describing their efforts to create inclusive communities in West Virginia. Presenters described how they engaged diverse stakeholders in a process of visioning & group action planning with the services of a graphic facilitator. Promising Practices in HCBS: Colorado- Simplified Access to Nursing Home Alternatives

The State of Colorado has established Single Entry Point (SEP) agencies that provide an access point for several publicly funded long-term supports for people with disabilities, including older people, people with physical disabilities, people living with AIDS, and people with brain injuries. SEP agencies provide education to individuals about available options and provide easier access for individuals and families to the programs. This report describes the SEP system in Colorado.

Request for Proposals - Ohio Access Success Project

The Ohio Department of Job and Family Services (ODJFS) released this Request for Proposals (RFP) for the purpose of identifying a vendor to develop and implement the Ohio Access Success Project. This is a nursing facility transition pilot project in which targeted individuals who are aged or cognitively, sensory or physically disabled and who have been residing in a nursing facility have an opportunity to choose to transition from that setting to a community-based one.
