This brochure is a marketing material for the program Passages in Arkansas. This program offers help to Medicaid residents in nursing homes who wish to return to the community.

The development of this Best Practices Manual is a pilot project to demonstrate a step-by-step process of re-integrating long term nursing home residents from residential living back into the community. This innovative Project is the result of an initiative made possible under the Passages Program sponsored by the Arkansas Division of Aging and Adult Services, in 2000-2003.

The Texoma Real Choice Program--a demonstration project made possible by a grant awarded to the Texas Health and Human Service Commission by the federal Centers for Medicare and Medicaid Services. The programs goal is to connect persons with disabilities to real service options with the help of system navigators available to assist people through systems and support groups. This Universal Design publication was developed by Kentucky Housing Corporation. This book reflects \"Tier One\" of KHC's implementation of it's Universal Design policy - this will be followed by the implementation of Tiers Two and Three. KHC refers to Tier One as \"Universal Design Light\", as it does not include requirements for all of the features that would normally be considered complaint with universal design principles.
