Transitioning Clients with Mental Illness from Colorado Nursing Facilities: A Best Practices Model Report

An advisory committee of consumers, providers, practitioners, advocates and experts worked collaboratively to identify the major challenges preventing people with mental illness from discharging out of nursing facilities and into less restrictive communities. This report is a result of their work and aims to make available a framework for successful transition programs. The best practices described are offered as a guide for service providers, advocates, and mental health clients themselves.

NewsCLIPS: Money Follows the Person - Fall 2006

HCBS.org News CLIPS highlights the important work of Real Choice Systems Change Grantees. In each issue, we examine a topic related to home and community-based care by providing an overview of recent papers and publications. The Fall issues provides an overview of the reports available on Money Follows the Person, a program that allows states to rebalance their long-term care support systems to support people in community-based alternatives rather than institutional settings.

Making Sure the Money Follows the Person in Your State Webcast

ILRU hosted a webcast where Bob Kafka, from ADAPT of Texas and Gwen Gillenwater from the American Association of People with Disabilities (AAPD) offered ideas about how policy planners, agencies, states and advocates can promote Money Follows the Person. There are a number of elements to consider including systemic issues, service coordination, housing, mental health, consumer involvement, quality and accountability. Real Choice Community Grant Request for Proposals and Questionnaire

WV Real Choice worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. One way the grant accomplished systems change at a grassroots level is by funding innovative community-based model projects. Review program materials including the RFP flyer and a project process questionnaire.

Navigating Youth to Adulthood: A Compilation of Projects and Activities Regarding Youth Transition

Transition can be defined as a continuing process toward independent adulthood. This paper illustrates the time line and products developed to foster improvements in the transition process for students with disabilities as partial fulfillment for the WV Real Choice Systems Change grant. In February 2005, an initial summit was held on youth transition to accurately assess the stance of educators, other professionals and parents. The results of this and future meetings can be found in this report.

Real Choice Systems Change Grant Training Final Report: West Virginia

This grant provided people with knowledge and resources concerning their rights and choices and educated key agencies throughout the state. This report provides an overview of grant activities include the Real Choice Speaker’s Bureau, training sessions and conferences. Trainings were conducted at over 40 Department of Health and Human Resource offices and 20 for the Division of Rehabilitation Services. Three conferences (Transitions, PATHS and People First) were also conducted.

Reaching Out Curriculum

The Real Choice Systems Change grant created a turn-key curriculum to promote inclusive community support for West Virginians of all ages and all disability types. Accessible curriculum addressing advocacy, community living, self-determination and transition was developed for the public, health care and social service providers, administrators and policymakers. A writing retreat was held to create the document and the outcomes and curriculum index were made available in electronic format.

Making Tracks to Transitions Program Resources

Schwab Rehabilitation Hospital has designed supports for youth with disabilities and their families as they transition from youth to adulthood. The three stage program creates a \"medical home\" for the youth age 14-21, creating relevant goals, and working with schools and community agencies to meet objectives. Resources included: initial assessment, checklists for the three areas of development (medical, psychosocial, and independent living), a parent checklist, and a bibliography.

ABC\'s of Nursing Home Transition

This self-study guide is offered as an orientation manual for new transition facilitators. Materials were developed with input from Centers for Independent Living and a variety of other organizations. A facilitator is somebody who aids or assists in the transition process. The manual helps new facilitators understand and apply the principles plus resources found useful by successful transition facilitators.
