This report describes the State of Wisconsin’s program to assist people with disabilities find housing. The state employs a supported housing specialist who works with local communities to help people with disabilities and their families to find affordable housing solutions. In addition, the supported housing specialist works with individuals with disabilities, local agencies, developers, housing providers, lenders,and other stakeholders to build relationships and improve housing access.

This report focuses on the Back Home in Indiana Alliance project. This project facilitates home ownership and rental by connecting people with disabilities and their families to a variety of housing organizations and resources. These organizations and resources include lending programs for low-income or under-served people, down payment assistance, grants for home modifications, and programs that link apartment seekers with appropriate, low-income rentals.

This document focuses on Sunrise Towers, a public housing complex for older people in Laconia, New Hampshire. This form of public housing is modeled after assisted living and provides meals and personal assistance to keep residents in the community. The document describes the program, the services the program offers, and funding sources.

Wisconsin uses a variety of initiatives to support Medicaid participants who want to leave nursing facilities, including ensuring services are available as soon as the person is ready to move and providing funding for transition. This document describes some of these initiatives.

The Formative Evaluation of the Real Choice Systems Change Grants evaluates how the Systems Change grantees are progressing towards meeting their goals. The Formative Evaluation identifies problems faced by the grantees and ways that they deal with the problems. This document provides summary information about each of the FY2003 Grantees.

This presentation will address a recent pilot study of the implementation of Rider 37 in Texas, the six pioneers and their support network that facilitated in the transition process from a nursing facility to the community, and the stakeholders in Texas who participated in the interviews.

In fiscal year (FY) 2004, Congress appropriated approximately $40 million in funding for a new round of Real Choice Systems Change Grants. With this solicitation, the Centers for Medicare & Medicaid Services (CMS) invites proposals for grants totaling approximately $31 million of these funds.

The Inventory of Community Services & Supports was designed by the Texas Nursing Home Transition project as a comprehensive profile of the requirements of an individual seeking to relocate from a nursing facility to a community residence. It was intended to be a working document, developed over time as decisions are made about the services and supports for an individual.
