This brief piece explains the steps involved in the transition process in Nevada including the services and supports available and the agency that provides referral for those seeking transition from an institutional setting. The goal of the project was to divert people from institutionalization and/or transition people from institutional care into a less restrictive environment.

The In-Home Supportive Services (IHSS) Program Consumer and Provider Handbooks

The IHSS handbooks were the product of many people’s efforts. The topics covered were determined by input from the IHSS Enhancement Initiative Task Force, focus groups of consumers, providers, social workers, and public authority staff in three California counties. The guide includes topics such as assessment, public authority, hiring, practical details like timesheets and benefits, communication and organization skills. Publications are also available in Armenian, Chinese, Russian, and Spanish.

Consumer Directed Attendant Support Program: Brochure and Application

The Colorado Dept. of Health Care Policy & Financing (HCPF) developed a program that enables people with disabilities to manage their own attendant services. In the CDAS, people hire, train, supervise and fire their own attendants. They set their own attendant schedules and to a degree they determine what services the attendants provide. HCPF is not accepting applications for the CDAS at this time however, the forms provide an overview of the program and serve as templates for similar programs.

Real Choice Mentor Training Manual

The purpose of the New York Real Choice Training Manual is to provide formal training to self-advocates who wish to become peer mentors. Mentor trainees will learn through the adult education model, which builds on the strength, capacity and experience of the learners. Self-Advocate consultants/mentors bring with them an incredible wealth of life experiences, which prepare them for this role.

Transitioning Alabama\'s Loved Ones Home

This report focuses on individuals with Alzheimer's and other dementias. The project instituted a comprehensive system to support enduring changes in the delivery of community-based assistance for Medicaid eligible persons with dementia. The projects two goals: facilitate hospital-to-home transitions for individuals wishing to stay in the community; and field test a nursing home-to-community transition program for eligible individuals. The workbook used in discharge planner training is included.

Real Choice System Change Grant Program: Fifth Year Report - Progress and Challenges of the FY 2004 Grantees (October 1, 2005 – September 30, 2006)

Consumer Survey for Use with Recipients of In-Home Services

The Oregon Department of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with HSRI to assist in improving the processes for assuring quality in SPD in-home waiver services. Drawing from HSRI's experience in quality assurance, SPD identified a number of states which have been particularly successful in conducting consumer surveys. At the same time, the project team explored in greater detail what was currently happening in Oregon around consumer surveys.

Long-Term Support Options Counseling: Decision Support in Aging and Disability Resource Centers

This paper presents the perspective that options counseling is a pivotal function of Aging and Disability Resource Centers (ADRCs) and its essential elements are counseling and decision support. The diverse array of definitions currently in use runs the risk of diminishing the importance of options counseling, which is a key function of a comprehensive long-term care system.
