This document identifies assessment instruments being used and reviews some of the variation across instruments and in the processes for using them. It includes an overview of the major components of the assessment instruments and of their role in a state’s eligibility determination, assessment, or care planning processes.

This monograph uses case studies to illustrate how specific cultural characteristics, such as language, customs and religion, can impact caregiving situations, and offers suggestions on ways cultural competency can be improved at organizational and policy levels. The purpose for this request for proposals (RFP) was to solicit proposals from qualified entities to provide under contract with the Department of Human Services a state-wide web-based online consumer information resource, personal assessment, and directory of services for people who are aging and people with disabilities, including developmental disabilities and mental illness.

Minnesota used this RFP to identify a contractor that could implement, host, and support an Internet-based long-term care database using a central web site with a companion desktop based client tracking system that will synchronize with the online database.

This report focuses on how to cover targeted case management for nursing facility transition activities. The report includes sample Medicaid state plan amendment language.

A transcript and summary of the primary options for covering case management as discussed during a call on 11/10/03 with Mary Jean Duckett are provided here. The three options states may use to cover case management under Medicaid that are relevant to NFT were discussed. These included: 1915 (c) home and community based waivers; Targeted cased management services; and Administrative activity.

Beginning in 2001 and continuing in 2002 and 2003, technical assistance for the Real Choice Systems Change grantees has been provided by the Exchange Collaborative team (combined efforts of ILRU and Rutgers University/CSHP). These documents provide contact information for the TA providers and two charts indicating which Exchange Team is responsible for providing TA to the grantees. In 2002 & 2002, TA was divided according to states. In 2003, TA was distributed according to theme.

This issue brief summarizes a follow-up study conducted with the executive staff of state boards of nursing (BON). The goal of the study was to explore the executive staff members’ understanding of consumer-direction trends and the extent to which their own policies support the desires of consumers to direct their own care tasks, that some may consider “nursing care”.
