Emergency Back-Up and Independence Plus Audio Conference

This transcript is from a call on \"Emergency Back-Up and Independence Plus\". Participants on the call discussed the requirement for states to have an Emergency Back-Up System at both the individual level and the systems level. Grantees heard examples from states in action, which included New Hampshire, Louisiana, Florida and South Carolina. These states discussed what they are doing around emergency back-up and some of the lessons that they have learned.

Alaska Consumer Assessment Tool (CAT)

The attached files are the instructions for filling out the Alaska Consumer Assessment Tool (CAT) and a copy of the tool. The form was designed to be an objective tool that is easily coded. Each subsection of the manual includes the “how to complete” instructions and the time frames in which to measure the person’s abilities. The design facilitates immediate eligibility determination by the assessor.
