Brief description of Georgia's experience in streamlining their financial determination process. Prepared by the Community Living Exchange Collaborative for the ADRC grantees.

This conference was designed to assess state initiatives to improve quality, cost, and access and to help policy makers—from both the executive and legislative branches—identify the most promising and cost effective of these programs and policies. This transcript is from a call on \"Emergency Back-Up and Independence Plus\". Participants on the call discussed the requirement for states to have an Emergency Back-Up System at both the individual level and the systems level. Grantees heard examples from states in action, which included New Hampshire, Louisiana, Florida and South Carolina. These states discussed what they are doing around emergency back-up and some of the lessons that they have learned.

Medicaid directors, Aging directors, and MR/DD directors in 5 states describe their experiences implementing and developing an IT system to support a \"single entry point\" (SEP), or \"no wrong door,\" service system. Approaches range from custom design to adaptation of \"off the shelf\" packages using vendors and/or state staff.

A booklet for individuals who want to leave a nursing facility and move to the community. This condensed booklet will give people basic information to plan their move to the community and get the needed supports.

The attachments are part of the Colorado Screening Tool ULTC 100.2 and include the Long Term Care Assessment For Instrumental Activities Of Daily Living Form And the Initial Screening and Intake Form.

The attached form is a copy of the North Carolina's assessment tool for the Community Alternatives Program for Disabled Adults program. There is also an example of Plan of Care form, AQUIP, provided.

The attached files are the instructions for filling out the Alaska Consumer Assessment Tool (CAT) and a copy of the tool. The form was designed to be an objective tool that is easily coded. Each subsection of the manual includes the “how to complete” instructions and the time frames in which to measure the person’s abilities. The design facilitates immediate eligibility determination by the assessor.

This document presents examples of presumptive and fast track eligibility programs implemented by several states to reduce the time it takes to process the Medicaid application.
