Incident Reporting Forms and Policies: Connecticut

The Department of Mental Retardation (DMR) has established a system of reporting and monitoring incidents that occur with individuals served by the department in order to identify, manage and reduce overall risk for the individual. This procedure delineates a standardized process for reporting, documentation and follow-up of reportable incidents as defined, involving individuals served by the department. This reporting system helps implement individual plans and assists in quality oversight.

2006 CMS Systems Change Conference: Access to Community Living

Request for Proposal and Certification Rules for Consumer-Directed Personal Care: New Hampshire

Granite State Independent Living (GSIL), working collaboratively with the Department of Health and Human Services (DHHS), solicited proposals from organizations interested in becoming consumer-directed personal care services providers (PSCP). Although this RFP is a few years old, it serves as an example of how to seek out providers. Moreover CHAPTER He-P 600 sets forth the procedures and requirements for operating as an other qualified agency.

Virginia Family to Family Health Information and Education Center Resources

Parent to Parent and Family Voices are proud to announce the new Family to Family Health Information and Education Center called the Virginia Integrated Network of Family Organization or The VA-INFO Center. The center provides youth and parents of children with special needs, service providers and others one place to call to get information and referrals, parent to parent peer support, annual family support conference, and other training opportunities.

Intake and Referral Information: Bangor ADRC

The Bangor Eastern Agency on Aging ADRC project has shared a number of resources involving the Intake and Referral Process. The first is their policies which provide staff guidelines for consistency. The second resource is their intake questions to help record and collect information. Lastly, they have shared their feedback referral process and how to make this simple and accessible by the consumers.

Improving Emergency Backup and Critical Incident Management for Consumer Direction

This report focuses on the current system for critical incident reporting and tracking within long-term care waiver programs. It also includes recommendations from stakeholders on a definition of critical incident and emergency backup, policy recommendations for new mechanisms that would more appropriately address the needs of consumers, and outlines an implementation plan for new critical incident management mechanisms and statewide emergency backup infrastructure.

Improving Infrastructure: Voices of Attendant Services Users

The Colorado Department of Health Care Policy & Financing contracted with an independent consultant to organize and facilitate six focus groups with consumers of Medicaid waiver services, family members or representatives, and consumer-directed program participants. The focus groups discussed consumer experiences with existing emergency backup and critical incident management systems and identified strengths and potential weaknesses. This report provides results from the focus groups.

Life Accounts and Asset Development: Linking Theory and Practice

A number of autonomy-compatible vehicles exist for people with disabilities and their families to exercise choice and control over resources; however, many programs are complex to navigate and vary from state to state. In shaping a savings program, benefits planning seems to be a likely ingredient. This report illustrates why LIFE Accounts holds great promise as an adjunct to other permissible approaches for the accumulation of assets that lead to increased independence and social participation.
