Emergency Backup & Safety and Prevention Strategies Brochure

This brochure was created using feedback from consumer focus groups, peer trainers, and other stakeholders. The brochure provides safety & prevention strategies, planning for backup care, preparing for attendant support during a community-wide disaster, how to minimize risk of theft, and health care emergency instructions and form. The strategies address emergency backup and incident management for consumers directing their own care or who have an authorized representative directing their care.

K-PASS Self-Direction Toolkit: A Comprehensive Guide to Assist Individuals in Self-Direction

The Shawnee County CDDO is pleased to present the Kansas Personal Assistance Supports and Services (K-PASS) Self-Direction Toolkit. The purpose is to provide people with disabilities the information and tools they need to hire and manage their personal assistant supports and services. The toolkit includes such items as job descriptions, training information, emergency back-up plans and other useful resources.

Pathways to Employment Conference: Final Report and Executive Summary

During the Pathways to Employment Conference in December 2006, the Ohio MIG program administered surveys and conducted three focus groups in order to assess the struggles and successes of consumers with disabilities in finding work, being in the workforce, and experiences with health coverage. A final report and executive summary were produced which recap and analyze the results of the surveys and focus groups.

MassHealth Managed Care Quality Strategy

This is the Quality Management Strategy for its 1115 Waiver covering managed care for non-elderly, non-dual MassHealth members. MassHealth, the state Medicaid Program, provides managed care services to eligible enrollees through its Managed Care Organization (MCO) Program, Primary Care Clinician (PCC) Plan, and the PCC Plan’s associated contracted behavioral health plan, and Senior Care Options (SCO). The Strategy incorporates the efforts and activities of several entities that serve enrollees.
