This brochure was created using feedback from consumer focus groups, peer trainers, and other stakeholders. The brochure provides safety & prevention strategies, planning for backup care, preparing for attendant support during a community-wide disaster, how to minimize risk of theft, and health care emergency instructions and form. The strategies address emergency backup and incident management for consumers directing their own care or who have an authorized representative directing their care.

The ServiceLink Resource Center has developed and shared a number of resources they are using to market the expansion of their service. Visit this ADRC’s website or review their outreach material which includes a fact sheet, newsletter guidelines, marketing plan and messages, advertisements for print and radio, a general press release, flyers, brochures and logo.

Colorado Real Choice Quality grantees designed this worksheet to guide clients as they \"shop around\" for the best provider agency to meet their needs. The worksheet suggests questions and criteria to use in evaluating agency services from a consumer perspective.

The Colorado Department of Health Care Policy and Financing received a Real Choice Grant for quality and created this brochure for consumers as part of their quality improvement activities. This tool provides strategies, resources, and guidelines for addressing home care concerns and, when necessary how to file complaints.

The Missouri Independence Plus Grantees have developed a number of tools during the implementation of their consumer-directed program aimed at helping people with disabilities. Review the 7 participant workbooks titles include Designing Supports, Financial Management, Individual Budget, Orientation & Overview, Person Centered Planning, Support Broker, Self-Determination. The Shawnee County CDDO is pleased to present the Kansas Personal Assistance Supports and Services (K-PASS) Self-Direction Toolkit. The purpose is to provide people with disabilities the information and tools they need to hire and manage their personal assistant supports and services. The toolkit includes such items as job descriptions, training information, emergency back-up plans and other useful resources.

During the Pathways to Employment Conference in December 2006, the Ohio MIG program administered surveys and conducted three focus groups in order to assess the struggles and successes of consumers with disabilities in finding work, being in the workforce, and experiences with health coverage. A final report and executive summary were produced which recap and analyze the results of the surveys and focus groups.

The State of Vermont was seeking proposals from individuals or organizations interested in becoming the Medication Assistance Consultant. The deadline for submission of proposals was June 23, 2006. This RFP serves as a great example when seeking out contract work.

This is the Quality Management Strategy for its 1115 Waiver covering managed care for non-elderly, non-dual MassHealth members. MassHealth, the state Medicaid Program, provides managed care services to eligible enrollees through its Managed Care Organization (MCO) Program, Primary Care Clinician (PCC) Plan, and the PCC Plan’s associated contracted behavioral health plan, and Senior Care Options (SCO). The Strategy incorporates the efforts and activities of several entities that serve enrollees.

How can you test the satisfaction of customers receiving services? The Forsyth County ADRC has shared a presentation about their process of measuring satisfaction. They have also included the results from their post-test consumer surveys and the findings that their questionnaires provided.
