Lifespan Respite Care Program

Grant awards totaling approximately $2.25 million to 12 states to implement the Lifespan Respite Care Program were recently awarded, adding to the twelve states that received awards in 2009. Lifespan Respite Care programs are coordinated systems of accessible, community-based respite care services for family caregivers of children or adults of all ages with special needs. A Toolkit for Serving Diverse Communities

AoA has released a toolkit that provides the Aging Network and its partners with a replicable and easy-to-use method for providing services for any diverse community. The toolkit consists of a four-step process and a questionnaire that assists professionals, volunteers and advocates with every stage of program planning, implementation and service delivery for older adult communities, their families and caregivers.

Community Innovations for Aging in Place (CIAIP)

CIAIP was authorized by Congress to assist communities in their efforts to enable older adults to sustain their independence and age in their homes and communities. Grantees are striving to assess their communities to identify barriers to aging in place and collaborate with other community organizations to identify innovative strategies for linking older individuals to programs that provide comprehensive and coordinated health and social services. What Works: Massachusetts Senior Benefits Expos

Discover strategies that were used to successfully engage local leaders and related agencies to host well attended and highly regarded Senior Benefits Expos across Massachusetts. Eight Expos were held across the state to inform and connect people to the programs and services they need, and provide them with the opportunity for on-the-spot screening and enrollment. The greatest success factor was believed to be the effective partnerships between federal, local and state agencies.

National Legal Resource Center (NLRC) Website

Visit this website, launched by the HHS Assistant Secretary for Aging. The Center was created to empower legal and aging services advocates with resources necessary to provide high quality legal help to seniors who are facing threats to their ability to live independently in their homes and communities. Community Living Program Fact Sheet

AoA launched the CLP initiative in the fall of 2007 with a vision to help states create person-centered programs that reach people at risk of institutionalization before they enter a nursing home. Since 2007, approximately $36 million has been invested in 28 states to develop the CLP program. With the implementation of the 2009 CLP grants, there will be more than 120 CLP program sites nationally. This fact sheet includes the program’s background, AoA’s vision for CLP and a list of the grantees.

Changing Systems, Changing Lives - 2010 AOA, CMS, and VA Grantee National Meeting

View presentations and handouts associated with this meeting held in Feb. 2010 in Alexandria, VA. Objectives of the meeting included increasing grantees' knowledge of successful strategies to propel their ADRC, CLP, and VDHCBS programs towards fully functioning status, showcasing best practices, promoting peer-to-peer exchange, building workforce capacity and encouraging successful partnerships for achieving sustainable reform. The conference featured a variety of plenary and workshop sessions.
