The attached files are the instructions for filling out the Alaska Consumer Assessment Tool (CAT) and a copy of the tool. The form was designed to be an objective tool that is easily coded. Each subsection of the manual includes the “how to complete” instructions and the time frames in which to measure the person’s abilities. The design facilitates immediate eligibility determination by the assessor.

It includes an overview of the major components of the assessment instruments and of their role in a state’s eligibility determination, assessment, or care planning processes.

This document highlights the Village Integrated Service Agency, a program of the Mental Health Association in Los Angeles County. Beginning in 2001 and continuing in 2002 and 2003, technical assistance for the Real Choice Systems Change grantees has been provided by the Exchange Collaborative team (combined efforts of ILRU and Rutgers University/CSHP). These documents provide contact information for the TA providers and two charts indicating which Exchange Team is responsible for providing TA to the grantees. In 2002 & 2002, TA was divided according to states. In 2003, TA was distributed according to theme.
