Findings from written survey of Employment Experiences and Focus Groups. The questionnaire sought to obtain information from individuals with disabilities about things that have helped them to get a job, barriers to employment, importance of medical benefits when making employment decisions, and how comfortable people feel disclosing their disability to an employer.

The A, B, C and D’s of Medicare

The manual is subtitled the Long-Term Care Options Counseling Tool Kit: A Professional’s Guide. It describe the four parts of Medicare Part A, B, C and D; describes who is eligible for Medicare Coverage; explains the roles of the U.S. Department of Health and Human Services, Social Security Administration and private companies as they relate to Medicare; and explains the concepts of deductibles, coinsurance and premiums.

Consumer Directed Attendant Support Program: Brochure and Application

The Colorado Dept. of Health Care Policy & Financing (HCPF) developed a program that enables people with disabilities to manage their own attendant services. In the CDAS, people hire, train, supervise and fire their own attendants. They set their own attendant schedules and to a degree they determine what services the attendants provide. HCPF is not accepting applications for the CDAS at this time however, the forms provide an overview of the program and serve as templates for similar programs.

Transition Guide to Community Living with a Self Assessment Tool

The booklet is a publication of the Louisiana Nursing Facility Transition Grant. It is a Self Assessment Transition guide to assist individuals in nursing facilities on how to start planning for a move to the community and obtaining the supports and services they will need. It includes contact information for available state programs, along with contacts for housing options.

FY2007 Real Choice Systems Change (RCSC) Grants Solicitation

This application offers a total of approximately $13 million in Real Choice Systems Change (RCSC) grants to qualifying States in two grant categories: State Profile Tool (SPT): Assessing a State's Long-Term Care System & Person-Centered Planning Implementation Grant. Persons with mental illnesses are noted as target populations for both grant categories. Applications are due by July 27, 2007.

Real Choice System Change Grant Program: Fifth Year Report - Progress and Challenges of the FY 2004 Grantees (October 1, 2005 – September 30, 2006)

This report describes grant activities (accomplishments and progress) in three major LTC systems areas: Access to LTC Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. The report also provides illustrative examples of the challenges Grantees have faced as they implement grant activities, and consumers’ roles in the implementation and evaluation of these activities.

Consumer Survey for Use with Recipients of In-Home Services

The Oregon Department of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with HSRI to assist in improving the processes for assuring quality in SPD in-home waiver services. Drawing from HSRI's experience in quality assurance, SPD identified a number of states which have been particularly successful in conducting consumer surveys. At the same time, the project team explored in greater detail what was currently happening in Oregon around consumer surveys.
