This report describes the design and implementation of IndependentChoices, Arkansas' model of Cash & Counseling. Lessons learned in Arkansas are discussed, and cover topics including outreach and enrollment, program features, counseling and fiscal services and program structure.

This technical advisory includes a description of the choice voucher system and prototypes for a Fiscal Intermediary Agreement, a Self-Determination Agreement, an Employment Agreement, a Purchase of Service Agreement and a Medicaid Provider Agreement.

This survey was developed for Michigan's Self Determination in Long Term Care program. The survey includes questions on fiscal services, hiring workers and enrollment.

Michigan has developed this information to clarify the model of Agency with Choice in their states. Agency with Choice is one model for supporting individuals in the MI Choice Waiver who are aged or have disabilities.

Self Determination in Long Term Care (SDLTC) is a MI Choice waiver program option. The materials included here were developed to market the program to individuals. Materials include an overview sheet, a Q&A sheet, an introductory letter, an information sheet, and a self assessment.

Contained here are materials used for the readiness review of the fiscal intermediary for Michigan's Self-Determination in Long-Term Care program. There is an Overview of the Readiness Review Protocol, the Readiness Review form, an Interview Protocol, and an Appendix for additional information.

Iowa has developed several marketing materials for their Consumer Choices Option. Materials include a one-page fact sheet, a brochure, talking points, a postcard, and an overview PowerPoint presentation.

This handbook explains the Consumer Choices Option available in Iowa. It explains the process of creating quality support services. The handbook talks about how you can have choices in your life-choices about where you live, where you work, and who you can count on for support.

This report showcases the findings of qualitative research that was conducted in Arkansas. The objectives of the research included: providing and overview of CCDE, identifying ways to increase referrals to the IndependentChoices program, and identifying professionals to act as ambassadors to the project. Research and findings are identified and discussed.
