This collection of materials comprises a short course for consumers to further peer support among individuals, to gain skills for use as Peer Support Specialists, to advance the use of warm lines and partnering activities, and to form and sustain peer support groups. The expectation is that people giving and taking this course will have heightened awareness of effective communication skills and resources to assist them as they provide peer support.

The federal government's one-stop Web site for people with disabilities, their families, employers, veterans and service members, workforce professionals and many others. The site connects people to information and resources needed to actively participate in the workforce and in their communities. Topics covered on the site include: benefits; civil rights; community life; education; emergency preparedness; employment; health; housing; technology; & transportation.

The purpose of this manual is to provide an overview of information and referral sources available to the Employment Liaison, individuals with disabilities, and student services staff at post-secondary schools. Resources in the manual reference both National resources and those specific to job seekers in Wisconsin.

This study was conducted as part of the Nebraska Nursing Facility Transition (NFT) Grant. The target population included children living in nursing facilities who meet the criteria for transition to home and community-based settings and children living at home identified as at-risk for nursing facility placement. The goals of this paper were to decrease the number of children in nursing facilities, identify risk factors, and to develop a plan to keep children in home and community settings.

The Consumer-Directed Supports Toolbox is a set of materials developed under the Community-Integrated Personal Assistance Services and Supports (CPASS) grant awarded to North Carolina in 2002. It was developed to support the implementation of consumer-directed options in North Carolina. The “toolbox” consists of 3 powerpoint presentations, a set of exercises to be used in conjunction with the powerpoints, a consumer-directed supports manual, brochures and other visual aids.

This survey developed by the Illinois Department of Human Services is intended to match potential roommates by compatibility of habits and interests. The goal is to find a good match of potential housemates.

The Oregon Dept. of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with Human Service Research Institute to develop a consumer survey exploring the satisfaction of people served by SPD in-home Waiver programs (seniors, people with physical and developmental disabilities). The project team worked collaboratively with SPD staff and Stakeholder Group to develop a survey instrument and process that adequately addresses the interests and concerns of all involved.

States' Initiatives to Address the Needs of Persons Dually Diagnosed with Psychiatric and Developmental Disabilities.

For the last two years, WV Real Choice has worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. Attached are the program's community outreach materials such as brochures, flyers, postcards and previous and current conferences advertising materials.
