This report describes the design and implementation of the Consumer-Directed Care Research Project (CDC) and draws lessons from the state's experience. The report is based primarily on in-person interviews conducted in January 2002, about 18 months after the project began enrolling beneficiaries.

The results of a survey on the interest of managed care organizations in developing a Cash & Counseling option for delivering long-term personal assistance services.

Review the key findings of this report which uses information from multiple state and federal data sources and provides the most comprehensive information to date on patterns of Medicaid and Medicare spending and service use among Buy-In participants. Overall findings suggest that Buy-In participants who are working may require fewer services or a less expensive mix of services than other adult disabled Medicaid enrollees.

Rhode Island’s Participant Guide includes general information about the program and details about roles and responsibilities. The guide includes sections on getting started with the program, completing spending plans, addressing personal issues, and managing emergencies.

These service standards were developed for New Mexico's Mi Via program. The document includes information about the eligibility determination process, support contractors, the budget allotment process, and participant rights and responsibilities.

Minnesota has an extensive media kit on their website about Consumer Directed Community Supports. The items in the media kit offers you some easy, ready-to-go materials in printed and electronic formats to use in promoting the consumer directed community supports service option among consumers, partnering organizations, and others. Materials include key talking points, a news release, a postcard, a kiosk card, a short quiz, and a brief description of CDCS services.

This report describes the design and implementation of IndependentChoices, Arkansas' model of Cash & Counseling. Lessons learned in Arkansas are discussed, and cover topics including outreach and enrollment, program features, counseling and fiscal services and program structure.

Two presentations are included here: "It is so Ordered: Modernizing Michigan Medicaid Long Term Care" and "Principles and Practices of Person-Centered Planning: Detroit Area Agency on Aging".

Michigan has developed this information to clarify the model of Agency with Choice in their states. Agency with Choice is one model for supporting individuals in the MI Choice Waiver who are aged or have disabilities.

Self Determination in Long Term Care (SDLTC) is a MI Choice waiver program option. The materials included here were developed to market the program to individuals. Materials include an overview sheet, a Q&A sheet, an introductory letter, an information sheet, and a self assessment.
