Consumer-Directed Supports Toolbox

The Consumer-Directed Supports Toolbox is a set of materials developed under the Community-Integrated Personal Assistance Services and Supports (CPASS) grant awarded to North Carolina in 2002. It was developed to support the implementation of consumer-directed options in North Carolina. The “toolbox” consists of 3 powerpoint presentations, a set of exercises to be used in conjunction with the powerpoints, a consumer-directed supports manual, brochures and other visual aids.

PASS Self-Assessment Tool for Consumer

This tool is designed to equip consumers with the tools necessary to develop a job description, to screen and interview potential employers (personal assistants), and to train new personal assistants as well as address consumers physical needs, leisure and community needs. There are over 30 checklist forms to help determine need. This publication was produced by the Tennessee Personal Assistance Supports and Services (PASS) program.

PASS Mentor\'s Guide: A Handbook for Mentors and Agencies

Through the Tennessee Personal Assistance Supports and Services (PASS) Program, people with disabilities will have the opportunity to self-direct the care and assistance they receive in order to live full, productive lives in their own homes and communities. This guide is designed as a handbook for agencies and mentors and offers practical information about the benefits of the mentoring process, describes roles, and offers training guidelines.

NewsCLIPS: Independence Plus- Summer 2006

HCBS.org News CLIPS highlights the important work of Real Choice Systems Change Grantees. Real Choice Systems Change Grant Program Fourth Year Report: Progress and Challenges of the FY 2003 and FY 2004 Grantees: (Oct. 1, 2004 – Sept. 30, 2005)

This report describes grant activities in three major LTC systems areas: Access to Long-Term Care Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. For each of the focus areas the report describes Grantees’ accomplishments, illustrates the challenges and discusses the consumers’ role in the implementation and evaluation of activities. The report uses information provided during the reporting period October 1, 2004 to September 30, 2005.

K-PASS Self-Direction Toolkit: A Comprehensive Guide to Assist Individuals in Self-Direction

Nursing Home to Community Program: A Discharge Planning Manual

The manual is a nursing home to community discharge planning manual designed primarily for use in nursing homes and may be of help to others to use in developing manuals for their communities. It is designed to orient the long-term care providers to the nursing home discharge process by defining the roles of the key partners who are involved in returning residents to community living and to document a standard protocol for accessing and maximizing partner resources.

Real Choice Systems Change Grants: Compendium Fifth Edition

The Compendium contains basic information about each of the Real Choice Systems Change Grantees. The Compendium will help you learn more about how these grants will be used to allow more people of all ages with a disability or long term illness to live and participate in their communities. Real Choice Systems Change Grantees will also find the Compendium useful to identify other Grantees with similar goals and activities. The fifth edition of the Compendium replaces all previous editions.
