Heightened Scrutiny Review of Newly Constructed Presumptively Institutional Settings

CMS released an informational bulletin that provides guidance regarding new constructions and the HCBS heightened scrutiny process. The 2014 Medicaid HCBS regulation’s settings regulation requires an in-depth assessment, called heightened scrutiny, for certain providers that are presumed to be institutional. This guidance provides clarification and flexibility to states, including an overview of three potential scenarios for how newly constructed settings could receive Federal funding

Designing a State-Based Social Insurance Program for Long-Term Services and Supports

The National Academy of Social Insurance released a report on potential designs for universal family care. The report highlights state program design proposals for early child care and education, paid family and medical leave, and long-term services and supports. The LTSS section of the report proposes four design issues states may face in developing a social insurance program for LTSS.

Evidence for the Impact of the Money Follows the Person Program

This report looks at how states with strong MFP programs compare to states without or with limited MFP programs. Findings show that states with more robust MFP programs reduced utilization of institutional LTSS at a higher rate than other states. In addition, these states also rebalanced their LTSS systems more efficiently compared with other states.

Mobility Managers: Transportation Coordinators for Older Adults, People with Disabilities, Veterans, and Other Members of the Riding Public

This paper describes the roles of mobility managers and explores the benefits of focusing on the individual and their unique transportation challenges. The report highlights emerging innovations from five case studies and presents the results of a national survey and inventory of mobility managers. It also discusses the future role of mobility managers in long-term services and supports.

Medicaid Home and Community-Based Services Enrollment and Spending

This issue brief highlights Medicaid HCBS enrollment and spending data from the Kaiser Family Foundation's 17th annual state survey. The report found that nearly all Medicaid HCBS enrollment (86%) and spending (93%) went to services provided at state option. This optional nature of most HCBS covered populations and services puts individuals at risk of losing services if the current structure of Medicaid financing were to change.

Strengthening Medicaid Long-Term Services and Supports in an Evolving Policy Environment: 2019 Update

This toolkit summarizes LTSS reform strategies adopted by state innovators. It identifies concrete policy strategies, operational steps, and federal and state authorities that states have used to advance LTSS reforms. It also includes specific case studies and advice for other states looking to implement similar reforms. In addition, CHCS has published a brief for state legislators looking for a summary of options for developing LTSS policies.

The Financial Hardship Faced by Older Americans Needing Long-Term Services and Supports

This study analyzes the financial burden experienced by people who require long-term services and supports. The study analyzes medical and LTSS spending among older Medicare beneficiaries, particularly the costs of assistive devices and personal care. The results show that for medical services covered by Medicare, beneficiaries with a high need for long-term services spend more than $2,700 a year out of pocket on average, twice as high as those without such need.
