Resource Guide for Health Care Teams

The University of California San Francisco (UCSF), National Academy for State Health Policy (NASHP), and John A. Hartford Foundation (JAHF) have put together a "Resource Guide for Health Care Teams". This resource guide includes Health Care Provider Tools of Engaging Caregivers, Caregiving Resources for Provider Referrals, and Billing Guides for Care Teams. The purpose of this resource guide is to offer health care professionals the tools and resources needed to include family caregivers in a patient's care journey which will help improve patient care and the caregiver's experience.
