The Long-Term Quality Alliance is pleased to announce the publication of five new case studies of exemplar programs that integrate long-term services and supports (LTSS) with medical care. This latest round of case studies included the following exemplar programs:

HealthPartners
Inland Empire Health Plan
Sunflower Health Plan
WellMed
These new case studies follow a first set of five case studies, published in April. Together, the ten case studies of exemplar programs, a working paper describing the key components to successful LTSS integration, and a taxonomy of LTSS integration, are part of a larger project to develop the business case for LTSS integration.
