Healthcare Booklet for Disabled Youth - California

Why is California preparing its disabled children for their transition to teen-hood? By engaging these children early, California hopes to get these kids involved in their own health care and future employment opportunities. The end goal is a transition to adulthood that is successful, fulfilling and without limitations. Early Considerations, Policies and Procedures – PersonalChoice – Rhode Island

VDHCBS Toolkit - Central Texas

Participant Goal Setting Tool

This tool was developed to help participants in Cash & Counseling programs set personal goals. There are three major objectives sighted for the process: To develop a structured process to ensure that the participants’ goals are clearly articulated and that the spending plan represents the needs and wishes of the consumer, to assure and improve quality at the individual level, and to use data collected for quality improvement at the program level.

Personal Care Services Eligibility Determination - Indiana

This is an analytical tool developed in 2009 by the National Consortium for Health Systems Development (NCHSD), in collaboration with CMS, to help states determine if they are eligible for a Medicaid Infrastructure Grant (MIG).
