Find helpful documents developed for the PersonalChoice program. Included are a flowchart of the referral/intake/enrollment process, Advisement Agency Certification Standards, a Training Manual and Quick Start Guide designed to give participants an overview of the program and their roles and responsibilities (including information on service and spending plans, communication tips, and information about working with PersonalChoice support agencies), and a Spending Plan template.

Review materials generated by the Central Texas ADRC providing information about the program, and many state-specific forms and documentation. Also included is a detailed Guidebook for participants in their Veteran-Directed Home and Community Based Services program. These materials (guidebook, sample budgets, emergencies, employment forms, job descriptions and trainings) can be used as templates for other states involved in similar initiatives.

Minnesota has an extensive media kit on their website about Consumer Directed Community Supports. The items in the media kit offers you some easy, ready-to-go materials in printed and electronic formats to use in promoting the consumer directed community supports service option among consumers, partnering organizations, and others. Materials include key talking points, a news release, a postcard, a kiosk card, a short quiz, and a brief description of CDCS services.

This presentation was part of the 2008 Home and Community Based Services conference in Boston, MA. The presentation details a toolkit available to address provider resistance.

This tool was developed to help participants in Cash & Counseling programs set personal goals. There are three major objectives sighted for the process: To develop a structured process to ensure that the participants’ goals are clearly articulated and that the spending plan represents the needs and wishes of the consumer, to assure and improve quality at the individual level, and to use data collected for quality improvement at the program level.

This Provider Resistance Toolkit includes: Tips on working with providers, a template letter for providers, template frequently asked question document, template Power Point presentation, sample case management references, graphic: Consumer and Provider tasks in consumer-directed services and basic components, and a sample presentation: Self-direction: What to look for from the federal, state and provider perspectives.

This is an analytical tool developed in 2009 by the National Consortium for Health Systems Development (NCHSD), in collaboration with CMS, to help states determine if they are eligible for a Medicaid Infrastructure Grant (MIG). This tool also includes links to more detailed information found in the 2008 MIG Solicitation document.

Four presentations were given on the effects of the Cash & Counseling Original Demonstration. Topics include quality of care, worker outcomes, Medicaid costs, and implementation lessons. In addition, a link to the webcast is available.

FCA launched this online community promoting professional knowledge around families in their caregiving role. The site offers resources to help policymakers, professionals and family caregivers develop caregiver-friendly legislation or initiatives, implement caregiver support programs or services, and access information to enhance the delivery of quality care. Interactive site features include an online forum and user submissions.

With the issuance of revised Ticket regulations, State VR agencies and ENs have expanded options for meeting the employment and training needs of Social Security beneficiaries with disabilities and new options for seeking financial compensation from SSA. This Toolkit provides information and tools that will help agencies successfully implement the Ticket program and take advantage of new options. It also provides examples to the new regulations and changes to VR Provider Handbook.
