The Delaware Health and Social Services (DHSS), Division of Developmental Disabilities Services (DDDS), requested proposals to select a contractor to provide consultation in the area of Quality Assurance/Quality Improvement (QA/QI) in Home and Community Based Services. The Request for Proposals was numbered NO. PSC617 and due by 08/13/04. Funds were available under a grant from the Centers for Medicare and Medical Services (CMS).

In fiscal year (FY) 2004, Congress appropriated approximately $40 million in funding for a new round of Real Choice Systems Change Grants. With this solicitation, the Centers for Medicare & Medicaid Services (CMS) invites proposals for grants totaling approximately $31 million of these funds.

CMS has announced a modification to its solicitation of proposals from states to develop Medicaid infrastructure to support the competitive employment of people with disabilities by facilitating targeted improvements to the state’s Medicaid program and/or developing a comprehensive employment infrastructure that coordinates disparate state service delivery systems. The purpose for this request for proposals (RFP) was to solicit proposals from qualified entities to provide under contract with the Department of Human Services a state-wide web-based online consumer information resource, personal assessment, and directory of services for people who are aging and people with disabilities, including developmental disabilities and mental illness.

Minnesota used this RFP to identify a contractor that could implement, host, and support an Internet-based long-term care database using a central web site with a companion desktop based client tracking system that will synchronize with the online database.

In a letter to State Medicaid Directors, CMS announced a funding opportunity under the Demonstration to Maintain Independence and Employment. This third-round solicitaion relects substantially more flexibility than what appeared in earlier rounds. This demonstration grant allows states to provide benefits, equivalent to those provided by Medicaid to the categorically needy, to workers who have physical or mental impairments that, without medical assistance, will likely result in disability.

The Department of Health in the District of Columbia used this template to find a contractor to operate a screening and counseling program regarding the long-term care (LTC) and chronic care needs for persons with a physical disability and elderly residents of the District. The operation of this program included developing and managing a centralized Resource Center. The District contemplated award of a cost reimbursable contract.
