Real Choice Systems Change and Aging and Disability Resource Center/Area Aging Grant Solicitation FY 2008

CMS has available approximately $8 million in funding to continue to support States' efforts to address complex issues in long-term care reform. In addition, CMS was also awarded $5 million for Aging and Disability Resource Center /Area Agencies on Aging grants. Applications are due no later than July 17, 2008. Please see the full solicitation for details. Also included for background is a list of FY2007 grant recipients focused on person-centered planning and state profile tools.

Medicaid Infrastructure Grant Announcement 2008

CMS was soliciting proposals to develop the infrastructure to support competitive employment opportunities for people with disabilities. Funding is intended to facilitate enhancements to state Medicaid programs and services, to promote linkages between Medicaid and other employment-related service agencies, and to develop a comprehensive system of employment supports. The demonstration period runs Jan–Dec 2008. This prior solicitation acts as a resource for terms, conditions and guidance.

Aging and Disability Resource Center Pilot Project Request for Proposals - Iowa

The Iowa Dept. of Elder Affairs was seeking proposals for development of two local ADRC pilot projects to provide a coordinated system of information and access in order to minimize confusion, enhance individual choice and support informed decision-making. Core functions include planning, collaboration, long-term services, supports & options counseling. Proposals were due on 3/20/08. Use this RFP as an example in developing similar products.

RFP for a qualified contractor(s) to complete the National State Profile Tool for Minnesota’s long-term system

The Minnesota Department of Human Services, through its Continuing Care Administration, is seeking Proposals from qualified Responders to work with DHS staff and key stakeholders to complete a State Long-Term Care Profile. A State Profile tool was developed by Thomson Medstat under contract with CMS. The Profile must describe the state’s long-term care delivery system for 7 population groups that account for the majority of people who need hcbs in MN. Proposals are due January 28, 2008.

Demonstration to Maintain Independence and Employment: 2006 Solicitation

CMS solicited proposals for this demonstration, created by section 204 of the Ticket to Work and Work Incentives Improvement Act of 1999. It allows states to provide benefits, equivalent to those provided by Medicaid, to the categorically needy, to workers who have physical or mental impairments that, without medical assistance, will likely result in disability. Development of Methods and Training for Assessing Personal-Experience Outcomes: Request for Proposals

Pilot Project: Real Choice in Support and Services

The goal of the Massachusetts Real Choice Pilot Project was to increase the quality of life and independence of participant through cost-effective flexible funding. Two pilot projects were developed under the grant to serve adults with disabilities. SAMHSA has just released three new Requests for Applications (RFAs) related to the promotion of mental health consumer and family member networking and technical support. These include funding opportunities for Statewide Consumer and Family Network Grants and Grants for National Consumer and Consumer Supporter Technical Assistance Centers. Real Choice Community Grant Request for Proposals and Questionnaire

WV Real Choice worked in collaboration with other agencies and organizations to create improvements in long term support systems so persons with disabilities and/or long term illnesses can make their own choices and have the necessary supports to live and work in their communities. One way the grant accomplished systems change at a grassroots level is by funding innovative community-based model projects. Review program materials including the RFP flyer and a project process questionnaire.
