Information & Assistance Handbook: Eldersource

It is intended for use as a guide for service providers to develop or enhance their own I & A processes or to replicate the system utilized at Eldersource. Housing Within Reach

Housing Within Reach is a project of the Tennessee Department of Mental Health and Developmental Disabilities. The project included a consumer driven longitudinal evaluation study across the state to document consumer perspectives on consumer housing preferences, and the availability, access and barriers to appropriate housing. The study included face-to-face interviews conducted with each study participant at baseline, and at six- and twelve-months.

Money Follows the Person - Getting It Done in Nevada

The purpose of this report is to provide recommended actions steps for the state of Nevada to establish structural, programmatic and funding mechanisms to facilitate a balance in resources for people with disabilities moving from institutions to community living. The primary principle of Money Follows the Person is simple and straightforward - people with disabilities should be able to decide where and how their services and supports should be provided.

Real Choice System Change Grant Program: Fifth Year Report - Progress and Challenges of the FY 2004 Grantees (October 1, 2005 – September 30, 2006)

This report describes grant activities (accomplishments and progress) in three major LTC systems areas: Access to LTC Services and Supports, Services, Supports, and Housing, Administrative and Monitoring Infrastructure. The report also provides illustrative examples of the challenges Grantees have faced as they implement grant activities, and consumers’ roles in the implementation and evaluation of these activities.

In Support of Oregon Children with Developmental Disabilities

The purpose of this study was to identify how DHS can use Medicaid and other resources more broadly and effectively to meet the family support and respite needs of Oregon families of children with developmental disabilities. Assess what SPD might do to improve its response to families through Family Support systems. This final report documents the activities, findings and recommendations associated with the study.

Consumer Survey for Use with Recipients of In-Home Services

The Oregon Department of Human Services, Division of Seniors and People with Disabilities (SPD) contracted with HSRI to assist in improving the processes for assuring quality in SPD in-home waiver services. Drawing from HSRI's experience in quality assurance, SPD identified a number of states which have been particularly successful in conducting consumer surveys. At the same time, the project team explored in greater detail what was currently happening in Oregon around consumer surveys.

Long-Term Support Options Counseling: Decision Support in Aging and Disability Resource Centers

This paper presents the perspective that options counseling is a pivotal function of Aging and Disability Resource Centers (ADRCs) and its essential elements are counseling and decision support. The diverse array of definitions currently in use runs the risk of diminishing the importance of options counseling, which is a key function of a comprehensive long-term care system.

Medicaid Insurance for Works with Disabilities Awareness Training

The Nebraska MIG program, in partnership with HHS, Easter Seals of Nebraska, and the Social Security Administration, held a training for the Medicaid Insurance for People with Disabilities program. The training included the following materials: flowcharts, research results, and PowerPoint presentations.

Strategic Plan 2006-2010: Alaska Works Initiative (AWI)

This strategic plan outlines the next five years, discusses goals and complements existing Alaska Works Initiative (AWI) activities to 1) address the major barriers that keep people with disabilities from working 2) enhance Alaska’s Medicaid programs and existing employment infrastructures to better meet the needs of working people with disabilities; and 3) imbed what has been learned through the AWI into a comprehensive employment system.
