Supported by a CES - Medicaid Infrastructure Grant, Minnesota developed a strategic plan to increase competitive employment of people with disabilities, called Pathways to Employment. The strategic plan describes how Pathways to Employment, through its partners and stakeholders, will work together to build a more comprehensive, coordinated system of employment services and supports for Minnesotans with disabilities. Additionally, the brochure and work plans for 2006 and 2007 are included.

This power point presentation and associated brochure helps Massachusetts families with children and youth with special health needs understand MassHealth, the MA Medicaid program. This program is an umbrella for many different types of public health care. Learn about eligibility, standards, criteria, and premiums. This work is part of the 2004 RCSC CMS Grantees for Health Care Information and Education Centers.

What programs do the MA F2F Information and Education Center offer? Review their brochure about the services and supports. This parent-run center provides free technical assistance about health care, public benefits and other health care financing options. The pamphlet is available in English, Portuguese and Spanish. See a variety of resources on assistance to people with disabilities in Michigan. Included are the Freedom to Work brochure, a Persons with Disabilities and Prescription Drug Coverage fact sheet, the Work Incentives Planning and Assistance brochure, Outreach Materials contact information, an on-line resources sheet, and a copy of the Michigan Freedom to Work for Individuals with Disabilities Law. The CHOICES CEO (Comprehensive Employment Opportunities) online newsletter, Working Together, focuses on Maine’s efforts to increase work opportunities for people with disabilities. Additionally, a brochure on MaineCare, a health insurance program for people with disabilities in Maine, includes information on benefits, qualifications, and local resources for the MaineCare option.

This photo essay captures the people behind the design of Oklahoma's Consumer Directed Personal Assistant Supports and Services (CDPASS) design. The Oklahoma Partnership, organized to support Oklahoma's Real Choice System Change and the CDPASS grants. It was also designed to engage Consumer stakeholders in an interactive environment.

Working Healthy, the Kansas MIG program, offers people with disabilities who are working or interested in working the opportunity to get or keep Medicaid coverage while on the job. The website offers an interactive map of benefit specialist, a chart on premiums (SRS Form ES-3165) , information on Work Opportunities Reward Kansans (WORK), publications, and FAQs. Also review their brochure in English and Spanish and their one page fact sheet used for outreach.

Indiana Works (Indiana’s Benefits Planning Assistance and Outreach Programs) received a MIG grant to conduct a statewide needs assessment in the disability community of the informational needs regarding state and federal programs and resources that support employment. The website contains general information about subjects and does not replace the expertise and assistance from trained benefits counselors. Also included for review are project overview and updates, fact sheets, and final reports.
