Fact Sheets for Consumers – South Dakota

How are states getting the facts out to consumers about their MIG program? South Dakota is providing readers with information about SSA Work Incentives, Taxes, Medicaid Buy In, Self Employment, Asset Development, Looking for a Job, Knowing Your Rights, and the Ticket To Work Program through fact sheets that outline the program and resources. Samples are attached and could serve as a guide to other MIG grantees.

The Sherlock Plan - Rhode Island

The Sherlock Plan is the name of Rhode Island’s MIG program. It allows qualified working people with disabilities to earn more income without the risk of losing vital health care coverage. To inform consumers about the program, they have provided a plan overview, fact sheets in Spanish and English, and more information on their "Rhodes to Independence" website. Samples of these documents and a link to the site are available.

Direct-Care Workforce – Federal Data

Find two fact sheets; the first presents occupational employment projections for the three job titles that make up the direct-care workforce: nursing assistants, home health aides, and personal and home care aides, and suggests that over the next decade, the nation’s direct-care workforce will constitute the largest, fastest-growing group of jobs in the country. The second provides a detailed overview of the demographic and economic characteristics of the direct-care workforce.

Implementing the Veteran-Directed Home and Community-Based Services Program

Employment and Disability Partnership Fact Sheet – Arizona

Looking for a clear and concise summary of an existing MIG grant program? Attached is the Arizona Employment and Disability Partnership MIG Grant Fact Sheet which outlines, among other things, the programs’ goal, its approach, 2010 planned initiatives, and its AHCCCS Freedom to Work evaluation and member survey. This document could serve as a helpful guide to other states contemplating a MIG program or accessing an existing program.

Asset Building Fact Sheet – Arizona

How is the state of Arizona facilitating the collaboration of it’s MIG program with the benefits planning community serving individuals with disabilities? The attached fact sheet was provided as a handout during a presentation with the asset building community meeting to solicit their collaboration in MIG initiatives. The handout identifies the commonalities between MIGs and asset building programs. It’s a collaborative example that could be helpful to other states looking to do the same.

Employed Persons with Disabilities Changes - Oregon

Because the MIG grant program is relatively new, flexibility is needed to help programs evolve successfully. One way Oregon has addressed changes in its program is by creating fact sheets. The attached example explains important changes in definition and introduces a new payment structure. It was provided to recipients, professional staff, stakeholders and the general public, and could be useful to other states with MIG programs.

Community Living Program Fact Sheet

AoA launched the CLP initiative in the fall of 2007 with a vision to help states create person-centered programs that reach people at risk of institutionalization before they enter a nursing home. Since 2007, approximately $36 million has been invested in 28 states to develop the CLP program. With the implementation of the 2009 CLP grants, there will be more than 120 CLP program sites nationally. This fact sheet includes the program’s background, AoA’s vision for CLP and a list of the grantees.
