This chart, developed by the North Carolina MIG, projects the estimated costs and enrollment in health coverage for individuals with disabilities in the workforce. The numbers are based on a cost of $1,210 per individual, per month and covers the five years of the program (2007-2012).

This strategic plan outlines the next five years, discusses goals and complements existing Alaska Works Initiative (AWI) activities to 1) address the major barriers that keep people with disabilities from working 2) enhance Alaska’s Medicaid programs and existing employment infrastructures to better meet the needs of working people with disabilities; and 3) imbed what has been learned through the AWI into a comprehensive employment system.

The Alabama MIG produced several action steps as a result of the resource mapping survey conducted in October and November 2006, including the action steps for implementing Medicaid Buy-In.

The Alabama Department of Rehabilitation Services (ADRS) Medicaid Infrastructure Grant developed this Logical Model to illustrate the collaboration project in the demonstration sites. The Model charts the phases of resources, activities, outputs, outcomes (short-term, intermediate, and long-term).

The New Jersey Department of Human Services’ Division of Developmental Disabilities (DDD) presents their action plan for systems change. This Plan outlines the process of transitioning individuals with developmental disabilities from developmental centers (DCs) to the community over the next 8 years (State FY 2008 through FY 2015). The Plan addresses the requirements of state legislation (S1090, PL 2006, Chapter 61) as well as the issues in the NJ P&A Olmstead lawsuit.

As part of the Georgia QA/QI Real Choice Systems Change grant, an evaluation of the Person-Centered Planning (PCP) program was conducted. Part of this includes the outline for planning and creating PCP groups, and a list of resources for effective use of PCP. Also included is the final survey given to PCP participants and information on sustaining person-centered planning.

As part of its quality management program, Wisconsin revised its Individual Service Plan (ISP) form that was required as part of the Medicaid waiver process. The new revised forms address individual outcomes. Attached forms include the ISP Instructions, Medicaid form, and Individual Outcomes.

This report provides a comprehensive framework for Wisconsin’s Functional Screen Quality Assurance. Wisconsin’s Functional Screen system (a web-based application that collects information about an individual’s functional status, health and need for assistance for various programs) is comprised of three separate screens: Adult Long Term Care Functional Screen (LTC FS), Children’s Long Term Support Functional Screen (CLTS FS), and Mental Health/AODA Functional Screen (MH/AODA FS).

The goal of the QAI Datamart is to develop a consumer-based reporting and data analysis database using the Home and Community-Based Services Quality Framework. The QAI DataMart will support data analysis and decision support for quality assurance and improvement. The purpose of this Software Requirements Specification (SRS) is to document the customer’s requirements for the applications being developed or enhanced. The reports and queries available from the DataMart are also included.

One objective of the Texas’s QA/QI grant was to develop a uniform and automated critical incident reporting process. Information was collected on critical incidents reported most often by other states to be used with the Quality Assurance and Improvement (QAI) Data Mart. The QA/QI task force reviewed and used this information to develop a list of critical incident definitions and recommendations.
